  14. Ruminant Gelatin BSE affected regions(Hide or bone derived) If: Country of origin of raw materials (hide and/or bone) is BSE free and country of processing/export is BSE affected, Then: Permit will require certification of country of origin and dedicated facility

  15. Ruminant Gelatin BSE affected regions(Hide derived) If: Country of origin of raw materials (hide only) is BSE affected, Then: Permit will require certification of hide derived and dedicated facility (excluding hides)

  16. Ruminant Gelatin BSE affected regions(Bone derived) If: Country of origin of raw materials (bone only) is BSE affected, Then: Permit will include affidavit requirements or importer responsibility restrictions

  17. Import Permit Application Process: • Complete the application (VS form 16-3) • Submit completed application by mail, fax, or on-line at www.aphis.usda.gov “Apply for Import Authorization” • Submit required user fees • Permit issued within 2-4 weeks • Permit duration is One calendar year
