Promoting MIYCN in communities through increased access to Safe Donor Human Milk

This project aims to increase access to safe donor milk through affordable pasteurization technology. The PiAstra, a low-cost device, allows for effective pasteurization without the need for a cell phone. The PiAstra has been successfully tested with milk from 45 donors and meets KZN HMB guidelines. The device is safe, user-friendly, mobile, and provides printable reports. It is being rolled out to community-based HMBs in KZN, rural hospitals in KZN and E. Cape, and hospitals in Ethiopia. This initiative is part of a larger effort to promote breastfeeding and improve infant feeding practices in communities.

  3. To increase access to safe donor milk we need to increase access to pasteurisation systems • But ……. Pasteurizers are expensive • Commercial Pasteurisers Cost R300,000 Plus

  4. A Brief History …………Flash Heating Pasteurization • PATH collaboration with: RohitChaudri, UW (Seattle), HMBASA and KZN-DOH • Developed a Simulated Flash Heating System • Temperature Monitoring • Display & Input through a Cell Phone • Hence known as FoneAstra

R&D work on the PiAstra • Comparison Temp Curves • Milk versus Water as the medium used for the temperature probe • A Variety of Cow’s Milks experimented with • Full Cream Long-life Cow’s Milk was found to have the closest heating properties to human milk • Glass bottles Best for Storing Breastmilk • However only metal lids available - Susceptible to Rust • Custom made plastic lid with perfect seal commissioned • Plastic lid autoclavable and long lasting

  8. R&D work on the PiAstra • Milk from 45 donors • Pasteurized with PiAstra • Post-Pasteurization - 2ml samples from each sample pasteurised, sent to NHLS for testing • Standard KZN HMB Guidelines Followed • KZN NHLS Culture on 5% Horse Blood Agar • All samples showed no bacterial growth

PiAstraStory extends …. • providing donor human milk it is not an end in itself – a means to promoting breastfeeding in communities • appropriate education and engagement around improving infant feeding practices and other public health practices needs to be encouraged simultaneously. • HIA is therefore also being used to train community members to develop Healthy Happy Families. Booklets provided to all.
