Step 1: Get the policy discharge form Your insurer will send you a Policy Discharge Form a month before your policy expires. The insurer will also tell you which documents need to be submitted with them. If you don’t want to wait for the insurer to give you the form, you can go online and download it from your insurer’s website. Step 2: Fill the form and enclose required documents The discharge form will require you to provide your name, the sum assured, your designation, address, bank account details etc. Once you are done filling up the form, you need to get it signed from two witnesses.
