This report reviews past grant initiatives in Michigan aimed at facilitating the transition of individuals from nursing facilities to community-based care. From 1998 to 2002, 174 individuals successfully transitioned, while 366 were diverted from facility placements. The Nursing Facility Transition Initiative (NFTI), administered by the Michigan Department of Community Health, partnered with various organizations, including the MI Choice Waiver Program and the University of Michigan. This initiative showcases the impact of collaborative efforts in promoting independent living for individuals with disabilities.
