Farmer’s Lung • Other names • Hypersensitivity pneumonitis • Extrinisic allergic alveolitis • Hypersensitivity alveolitis
Farmer’s Lung • Causes • Breathing in certain allergy-causing dusts such as that from moldy hay • Also from any other moldy crop such as tobacco, straw, corn, silage, etc.. • Aspergillus most common mold • Micropolyspora faeni and Termoactinomyces vulgaris most common bacteria
Farmer’s Lung • Epidemiology • Farmers, esp dairy farmers late winter or early spring in wet weather regions • Clinical signs • General malaise • Breathing problems sim to pneumonia • Acute, subacute or chronic presentation
Farmer’s Lung • Prevention • Dry crops before storage • Work in ventilated areas or outdoors • Wet down dusty areas prior to working in them • Wet hay should be ensiled—key to stopping mold growth • Consider respirator

Silo Filler’s Disease • A disease caused by the inhalation of nitrogen oxides • Toxic levels of NO, NO2, N204 are all produced in silos • These gases are dense and settle in the chute and base of the silo • NO2 is brown and has an odor • N204 is colorless and odorless • When inhaled, they make nitric acid
Silo Filler’s Disease • Clinical signs • Mild exposure: cough, chest tightness • Large exposure: vascular collapse, shock, death • Other sources of nitrogen oxide gases: • Furnaces, diesel fumes
Silo Filler’s Disease • Prevention • Do not enter a recently filled silo • Wait at least 2 weeks to enter • Possible toxic exp if silo unopened even after 2 weeks • Allow silo to “air out” 20 mins before entering • Wear PAPR respirator • Know corn, oats, heavily fertilized crops more likely to yield nitrogen oxides
