Extradural haemorrhage • The most likely brain bleed to cause a sudden death from a car crash • Undetected but suddenly becomes critical and the patient can have a seizure or lose consciousness. • An acute haemorrhage between the dura matter and the inner surface of the skull. • Can cause compressions of the brain structures and a rise in intercranial pressure. • If pressure continues to rise it could cause brainstem death. • Brainstem death is when the brain is no longer passing messages to the body so the body cannot breathe unless on a ventilator. The patient will lose consciousness. It is irreversible. These patients are declared brain dead. If this happens the patient will quickly lose consciousness and stop breathing.
What would happen if a patient has an Extradural Haemorrhage during surgery. • Blood pressure monitor will start to beep. • Surgeon would say something like: ‘Blood pressure is climbing what is happening? Must be a brain injury. Did they have a CT when they came in. Let me check their pupils.’ • Checks pupils with a pen torch. There is no response to light and pupils are dilated. • Surgeon would say something like: ‘There must be a bleed we need to evacuate the clot has anyone got a drill. Quickly we are losing them! • Heart monitor has one long continuous beep as there is no electrical activity in the heart. • ‘Time of death …’ Heart monitor flat lined.
