  8. Extradural haemorrhage • The most likely brain bleed to cause a sudden death from a car crash • Undetected but suddenly becomes critical and the patient can have a seizure or lose consciousness. • An acute haemorrhage between the dura matter and the inner surface of the skull. • Can cause compressions of the brain structures and a rise in intercranial pressure. • If pressure continues to rise it could cause brainstem death. • Brainstem death is when the brain is no longer passing messages to the body so the body cannot breathe unless on a ventilator. The patient will lose consciousness. It is irreversible. These patients are declared brain dead. If this happens the patient will quickly lose consciousness and stop breathing.

  9. What would happen if a patient has an Extradural Haemorrhage during surgery. • Blood pressure monitor will start to beep. • Surgeon would say something like: ‘Blood pressure is climbing what is happening? Must be a brain injury. Did they have a CT when they came in. Let me check their pupils.’ • Checks pupils with a pen torch. There is no response to light and pupils are dilated. • Surgeon would say something like: ‘There must be a bleed we need to evacuate the clot has anyone got a drill. Quickly we are losing them! • Heart monitor has one long continuous beep as there is no electrical activity in the heart. • ‘Time of death …’ Heart monitor flat lined.
