Identify the factors that contribute to the difficulty: Disability, Lack of instruction, LEP, Response to Intervention, Medical, and Environmental factors.
Student is not eligible under Chafee Amendment
Identify additional instruction, assistive technology, supports, services, and/or training that will be needed by the student and others to use the materials effectively.
