  1. Difference between Fissure and Piles: Tips to know Piles or hemorrhoids are inflamed arteries near the anal opening. A fissure is an injury in the thin skin of the anal canal. Depending on how bad the disease is, piles are scored from 1 to 4. In grade 1, only the blood vessels are swollen, while in grade 4, a mass comes from the opening.
