Here's an example of interview questions we used to test our hypotheses around professional feedback in the workplace.

Problems Hypothesis 01. Lack of feedback culture (not a habit or established process) 02. People do not know and/or do not understand what feedback is for 03.People think that giving feedback is difficult or time-consuming, they don't know how to write it 04. People do not know how to convey negative feedback so that it changes the situation for the better
01 Questions of Context How is the process of employee reviews/collecting feedback organized in your company? 02 Tell me about the last time you provided feedback to someone With all the details: to whom (role)? positive/negative? verbal/written? spontaneous/planned by both sides? what were the next steps after it? 03 Tell me about the last time you received feedback With all the details — was it comprehensive? 04 Were you satisfied with how it happened?
Questions for Problem 1 Are you satisfied with the current feedback process at your company? Yes No Are you trying to improve it somehow? What difficulties did you face? What’s the hardest part about that? That's great! But can you remember any difficulties that you faced?
Questions for Problem 2 01 Do you receive feedback from your manager? And colleagues? 02 How often does this happen? 03 Are you satisfied with the way it's organized now?

(case by case, once per year) 04 Do you share negative feedback? How often? Do you track progress after your feedback?
