  1. DEFINITION Lowbackpain(LBP)isoneofthemostcommonreasonsforavisittoyourphysician,secondonlytothecommoncold.Itcanbecausedbyanynumberofreasonsincludingpoorsittingposture,heavylifting,orfrequentbending. IfyouhavehadLBP,youmighthaverealizedthatalotofpeoplehaveexperienceswithbackpainjustlikeyou.ThatalsomeansthatmanypeoplehavetriedmanydifferenttreatmentsandmodalitiestohelpdecreasetheirLBPandimprovetheirfunction. Aninversiontableisapaddedtablethatisconnectedtoametalframewithhinges.Tousetheinversiontable,youstrapyourselfontothetableandslowlyallowthetabletoflipover,thusinvertingthebody.Inversiontablestypicallyrangeincostfrom$100to$400.Thesimiliardevice,called“Gravityboots”,waspopularinthe1980s. Anothernameforinversiontablesorinversiontherapyisgravitationaltraction.

CLAIMS Thetheorybehindinversiontableuseisthatwhenthebody'sweightissuspendedfromthelowerbody–ratherthanborneonthehandsasinhandstandsorheadstandsorhangingfromabarwitharmsatsides,whicharealsoformsofinversion–thepullofgravitymaydecompressthejointsofthebodybelowtheanchor.Hangingbythefeet,aswithgravitybootsorinversiontables,causeseachjointinthebodytobeloadedinanequalandoppositemannertostandinginanidenticalpositionofjointalignment.Thisisthoughttocreateatractionforcethroughthespine,andithasbeencommerciallyadvertisedasareliefforbackpain

  3. RISKS Peoplewhohaveheartdisease,highbloodpressure,eyediseases(suchasglaucoma),orarepregnantareathigherriskforthedangersrelatedtoinversiontherapyandshouldconsulttheirdoctorsaboutitfirst. Thefirsttimeanyonetriesinversiontherapywithgravity,theyshouldbesuretohavesomeonestandingby,incaseassistanceisrequiredtogetoutoftheapparatus,orifhealthproblemsareexperienced. PeoplewithGstroesophagealrefluxdisease(GERD)arealsoatrisk.Duringanepisodeofacidreflux,smallamountsofstomachacidmanagetoescapeoutthestomach'sdoorwayandintotheesophagus.Fortunately,gravitykeepsmuchofthestomachacidawayfromthisdoorway. However,inaninvertedposition,gravitycannotdoitsjob.Combininganinversiontableandacidrefluxcanbeapainful,nauseatingandpotentiallydangerouscombination.

EFFECTIVNESS Moststudiesindicatethatinversiontherapydoescausesometractionforcethroughthelumbarspine.Onestudyfoundasmuchasa3mmseparationbetweenlumbarvertebraeduringinversiontherapy.Sothequestionarises:Doeslumbartractionhelplowbackpain? Areviewofavailableresearchwaspublishedin1995andfoundthatmoststudiesabouttheefficacyoftractionfor low back painwereofpoorquality.ThosestudiesthatwereofhighqualitywerenotabletodemonstratethatlumbartractionhelpsLBP.Conclusion:WhilelumbartractiondidnotseemdetrimentaltoindividualswithLBP,italsodidn’tseemtohelpmuch. Apaperinthe2001issueofPhysicalTherapyJournalexaminedpublishedevidenceforvarioustreatmentmodalitiesforacute(12weeks)non-specificLBP.Inshort,thefindingsindicatethattractionforacute,sub-acuteandchronicLBPreceivedagradeof"C"(nobenefitdemonstrated).Todate,nootherhighqualitystudieshavebeenpublishedtorefutethesefindings. Thesamepaperinthe2001PhysicalTherapyJournalthatreviewedLBPconcludedthatthebesttreatmentfornon-specificacutelowbackpainistoremainasactiveaspossible.Forsub-acuteandchroniclowbackpain,theevidenceshowsthatexercisereceivedagradeof“A”(benefitdemonstrated).Thepaperconcludes:“Thereisevidencetosupportandrecommendtheuseofcontinuednormalactivitiesforacutenon-specificLBPandtherapeuticexercisesforchronic,subacute,andpostsurgeryLBP.”1

  5. CONCLUSION Therearenoclearindications,however,thattractionisanineffectivetherapyforbackandneckpain.Furthertrialsmayproveothervise. Butifyouaresufferingfromlowbackpainorpaininthelegthatiscomingfromtheback(sciatica),thebesttreatmentforyoutoengageiniscontinuenormalroutinesandperhapsaddexercise.Theuseofmechanicaltractionseemstoofferlittleornobenefitforlowbackpain. Comparingtherisksassociatedwithinversiontables(glaucoma,bloodpressurechanges,heartratechanges)withthebenefitsexpectedwithusinginversionforlowbackpain,itwouldseemthatyourtime(andmoney)wouldbebetterspentexercisingtohelptreatyourpain. Avisittoyourphysicaltherapistisareasonableplanofcareforyourlowback.Heorshecanprescribeexercisesforbackpainandhelpyouchangeyourposturetohelpyourcondition.Yourphysicaltherapistcanalsoteachyouwhyyourbackishurtingandcanhelpprovidestrategiestopreventfutureproblemswithyourlowback.
