  15. Dominion VA Power Near Miss Workflow Any employee can search for published reports by date, keyword, and several other factors. Causal Codes do not appear. Any employee enters a “Field Survey” via the on-line survey or via printable form. Completed survey does not get published yet. (holding tank). 1 2 3 4 • All four coders get an email flag that new survey is ready. The Coder on duty for that report (Jake=0-3, Bill=4-6, etc.) • ensures it’s legitimate • sterilizes it of any identifiable info • tags it with Causal Codes (in development) • If needed, sends it out as an “urgent” action” alert • gets one other Coder to peer check it • publishes it (ideally within 24 hours), and • destroys the original survey to ensure anonymity. • If time is tight, coder can skip step 4 until convenient. About monthly, coders collect the best stories of that month and publish a 1-2pp newsletter highlighting “Near Misses of the Month.” (Modeled after ASRS’s “Callback” newsletter.)
