  1. Human Safety whileworking at Track/Work spot Case study (1. Personnel Safety)

Case No 1. • On Date 21/08/2007 while working on track for connecting line no 4 ,West end of Uslapur OHE staff also working at same location at 17.30 Hrs • Suddenly rain started and OHE staff Left site and got down from OHE mast. • Satff who was climbing on two mast away Cut The OHE Wire without any warning.

  3. Case Study Uslapur Yard West End • Visibility condition suddenly Impaired and OHE Wire Fallen on the Track • Sri B.C. Manna SSE (P.Way) Const. Who was supervising the Track connection work Got serious Injury In His Head His Life was only saved due to Umbrella stick By The grace of God.

  4. Lesson • 1.During multi Departmental work Close Co-ordination to Be kept with each Other. • 2. Helmet and other protective clothing's to be provided to all workers ,Present at work site.

  5. Case No 2(L.C. accident) • In year 1989 Morcha L.C. Bet GGT-MDXR at Km 935/15-17when 407 Up Passenger was crossing The L.C. One bus carrying 56 passenger coming from Birsinghpur and Suddenly dashed The Barrier at same time and without any Time gap Hitted the engine of 407 UP.

  6. L.C. Accident 2.Driver of passenger train make his best possible effort and applied emergency brakes but due to sudden events of Barrier breakage and there is no gap in between Barrier breakage and arriving of passenger Train , Engine Stopped after Passing 250 m Distance at L.C.

  7. L.C. Accident • Bus was completely Distroyed and Burnt Into Fire and All 46 Passengers with driver Got death. • L.C. gate keeper closed the Barier & Locked The gate after getting the Information of 407 UP passenger

  8. Probable Cause • Due to Driver Expired at site and all the parts of passenger completely Crushed under the engine It was expected that sudden failure of Brakes of Passenger Bus is only the cause of This accident.

  9. Lesson – • Before Starting any motor vehicle Driver Should Test His Brake on first Falling Grade . In Case of any Doubt He Should Stop his Motor Vehicle till rectification of defects noticed,. • Driver should not Ignore small defects and Take suitable action for rectification.

  10. Case No 3 ( Staff Run over) • In Year 1989 In Between CHD-LOA when one Dn Loaded Train Stalled In Between CHD-LOA at Km 990/11-13. • It was decided By Sectional Controller To Introduce Single Line Working for Passing 8233 DN By Up Line for saving detention.

( Staff Run over) contd,. • G.No 73 was working T.P.on Up Line Between Km 888/13-15 under the supervision of Akali Mate. • When 8233 Dn approached The worksite the gang was doing the alignment rectification work and mate was sighting the rail ,another Trackmen were slewing the track with crow bar.

( Staff Run over)Contd. • The work spot was situated with a 2.5 m deep cutting and 1 Degree LH Curve. • The Mate could not judge The Approaching Train which was in high speed resulting 4 nos Track man was runover and spot dead and other Trackmen Narrowly escaped and could hardly saved their life

  13. ( Staff Run over) Probable Cause • After Investigation It came to Notice that This Accident took place By Issuing wrong caution order by S.S. CHD On duty to driver of 8233 DN. • SS CHD should Issue caution order To The Driver of 8233 DN That your train is First Train Going on T.S.L working , Make long whistles , Inform Gate man and Gangman accordingly But He failed to do so.

( Staff Run over) Contd. • Further it came to notice of Enquiry Committee That this actually happens due to Assigning of work to another SM &Issuing of Caution order & Authority To Proceed By Another S.M. who actually performed the Night Duty on previous day and arriving SS office Due to personnel work. He Issued the caution order & Forgot to mention the speed & Instruction to 8233 DN That your train is First train Inform Gateman & Gangman accordingly & Also the speed restriction also which should be followed BY First train going on wrong line in case of T.S.L.

  15. ( Staff Run over) Lesson • 1. Nobody should assign safety related work to another person. • 2. In case of introducing TSL Proper care should be Taken while Issuing caution order to First Train as per G.R. • 3. In case of work spot on Poor visibility movement of Track from Both Direction should be observed care fully • 4. Look out man with hand signals to be posted. • 5. Observation Post must be erected at such location from place where visibility Impairing starts.

  16. Human Safety while on track Machine(averted accident) • When Unimat machine was working under Block protection at west end of SNGP Yard Up Line points No W/12E in Year 2000. Three Nos Staff & Technician was deputed to observe the abnormality of Tamping unit and other moving parts of Machine. At 8.30 Hrs • On Dn Line side visibility is restricted Up to 400 meter so a Flagman was also kept to warn the approaching train. • Meanwhile Lookout Flagman Waived H..S.Flag For indicating the information of approaching train.

  17. Human Safety while on track Machine(Case No –4) • The Three staff who was standing on Dn Line Track due to Close Track Centre , Failed to take the Notice of approaching train and could not hear blowing whistle of 8478 Dn. • I was also standing Just 10 Meter Rear of Track machine, When I Noticed That staff of track machine can not notice The whistle of approaching train and may be runover then I Jumped and pushed them Towards the Up Line track side Just before passing the Engine of 8478 Dn.

  18. Lesson • While working on the track with track machine oftenly the noise created of Engine , Tamping unit mixed , and the staff Just side of tamping unit may Knocked/run over by Train approaching on Up Train.( Due to not hearing the whistle of train coming from another adjacent Track/line)

  19. Lesson • In case if machine working on Locations of restricted Visibility Portable W/L Board should be fixed at adjacent Line with a caution order of Obey Engg. Signals.

  20. Safety of work Spot Normally Done In Proximity of running track Case No5) • Such Types of work includes • 1. Cutting widening/bank repairing/Construction of New Bank by use of Heavy Earth Movers • 2. Ballast Stacking/ Carrying of heavy materials By Truck , which are plying near railway track • 3.Erection/Repairing /Dismantling of new Structure • Demolition of abandoned structure/Passing over the track(like F.O.B./R.O.B.)

Safety Of work spot co-related with work to be done in Proximity of running tracks • Though In our division no such type of accident took place,( Except averted accidents) But a lot of such type of accident occurred in other divisions/Railways During Dismantling work of abandoned structure However the following measures are Suggested from my ends.

  22. Safety Of work spot- ---Proximity of running tracks • 1(i) The contractor shall not start any work without the presence of railway supervisor or his representative and contractors supervisor at site. .

Wherever the road vehicles and/or machinery are required to work in the close vicinity of railway line, the work shall be so carried out that there is no infringement to the railway’s schedule of dimensions. For this purpose the area where road vehicles and/or machinery are required to ply, shall be demarcated and acknowledged by the contractor

  24. Safety Of work spot- ---Proximity of running tracks 3. Special care shall be taken for turning/reversal ofroadvehicles/machinery without infringing the runningtrack. Barricading shall be provided wherever justified and feasible as per site conditions.

  25. Safety Of work spot- ---Proximity of running tracks 4. The look out and whistle caution orders shall be issued to the trains and speed restrictions imposed where considered necessary. SuitableFlagmen /detonators shall be provided wherenecessary for protection of trains. 5. Supervisors should be counseled about safety measures & Competency certificate should also be issued.

Safety Of work spot- ---Proximity of running tracks • 6.The unloaded ballast/rails/sleepers /other P.Way materials after unloading along track should be kept clear off moving dimensions and stacked as per the specified heights and distance from therunning track. 7.Methodology Proposed to be adopted shal be approved by Engineer In Charge.

  27. Safety Of work spot- ---Proximity of running tracks • 8. Supplementary site specific Instructions should be issued wherever considered necessary. • 9.There should be an assurance register signed by both Contractor & Railway representative.

  28. Conclusion • After all The Rules for execution of work & Protection of Work spot is so Strong ,that it is foolproof in itself only. • And follow the Rules of safety for work spot is not difficult. • Safety Consciousness must be imparted through regular session, Interaction,and field drills. • “ Safety First” should be followed
