  2. Objectives • At the end of this module you will: • Understand how and why patient participants will keep a journal during this study • Be able to describe how community health workers will personalize each participant’s journal and collect these journals from participants on a monthly basis

  3. Why Journals? • It is a concise way to remind participants of when they will be contacted and what they need to send to the study team • Daily reminder and systemic way to gather out-of-pocket cancer costs • Provides a location to gather feedback from the participant about their experience in the project

  4. Journal • Date book with reminders for • Appointments with the CHW • Saliva Sample collection days • Recording daily out-of-pocket costs associated with cancer treatments • Open space for notes • Prompts about the research experience

  5. Sample Reminders • Tomorrow you will collect three saliva samples • Today your CHW will call between 9:00 AM and 11:00 AM. • Please take time today to review your out- of-pocket expenditures and enter them on the Out-of-Pocket Expenses grid.

  11. CHW Responsibilities • Set following month’s phone call time and date with participant by 18th of each month • Communicate the dates and times to the Research Office • Generate and mail post card reminder about completing and returing journal to participants around the 15th of each month • If participant reports misplacing or losing journal print and send additional copy to participant
