  2. About Shared Living • Is an Adult Foster Care (AFC) Program at Boston Senior Home Care, for elderly and disabled individuals unable to live alone in the community • A caregiver is financially reimbursed to provide daily assistance in order to keep the individual safely at home

  3. Eligibility • Must be eligible for Standard MassHealth • 16 years of age or older • Needs daily physical assistance with at least one activity of daily living (ADL) such as bathing, dressing, toileting, transferring, walking or eating • be under the care of a physician • Identify a Primary and Secondary caregiver or be willing to be matched by our staff

  4. Primary and Secondary Caregivers • The Primary Caregiver and consumer must live together in the same unit with their own bedrooms • The Primary Caregivers provides 24 hour supervision • A Secondary Caregiver must also be identified, to provide care in the absence of the Primary Caregiver • The Secondary Caregiver does not need to live with the consumer

Home Requirements • A private residence or apartment with a safe, clean room with a door and window • Primary Caregiver and consumer must live together in the same unit with their own bedrooms • Occupied by the caregiver or consumer family and no more than 2 people for whom care is being provided • Clean and in good repair with appropriate safety equipment in working order

  6. Adult Foster Care offers two levels of care • Level I is for people who need daily assistance with at least one of the following activities of daily living (ADL): bathing, dressing, toileting, transferring from one position to another, ambulating (walking or wheelchair assistance), or eating • Level II is for people who need assistance with three or more ADL’s or two or more ADL’s, plus caregiver intervention for one or more of these behaviors: wandering, resisting care, being physically or verbally abusive, or socially inappropriate or disruptive behavior.

  7. Primary Caregiver Responsibilities • Provide 24-hour supervision and physical assistance necessary for the consumer’s well being • Comply with the consumer’s Care Plan written by the Shared Living Registered Nurse and Case Manager following their assessments • Assist with arranging and transporting the consumer to health-related appointments • Provide 3 nutritionally balanced meals and snacks daily

  8. Caregiver Responsibilities cont’d • Keep records and report changes in consumer’s condition to the Shared Living Team • Maintain a safe, clean home with in good repair • Maintain a monthly Caregiver log provided to the Primary Caregiver during orientation by AFC staff

  9. Who can be a Caregiver? • Caregivers can be family members or non-family members. However, caregivers cannot be spouses, parents of minor children, or “legally” responsible relatives. • Caregivers must be approved by the program through interviews, letters of recommendations, a CORI check for all living in the household, and must be able to provide 24-hour supervision. • Caregivers must provide proof of a clean bill of health

What benefits will I get? • You can live in your caregiver's home, or your caregiver can move into your home • If you need a caregiver we can match you with one • If you would like to become a caregiver we can match you You will be treated like a member of the family by your caregiver Your caregiver will: • prepare meals and snacks • assist you with personal care • provide behavior interventions as needed • provide medication reminders and assistance • conduct grocery shopping, laundry, and housekeeping • provide transportation to medical appointments and social visits • maintain a clean safe living space • keep a caregiver log

  11. Your Caregiver will receive: • up to $1500 per month for caregiver services ($18,000 per year) for Level II consumers, and $600 per month for caregiver services ($7,000 per year) for Level I consumers (on a tax-free basis) • support and training by AFC staff • home visits by Case Managers and Registered Nurses • respite care (up to 14 days per year) Your caregiver is not allowed to have more than 3Level I consumer’s at a time, and no more than 2 who require Level II care.
