Next Steps towards Addressing the Key Determinants of Health • Finalize the Community Health Indicators to track and report on • Complete the Community Health Needs Assessment • What assets exist already • What gaps do we need to address • Prioritize those needs with key considerations • Identification opportunities for collaboration/partnerships • Implement technologies to support patient information is accessible at the right place at the right time across ALL sectors of services • IMMEDIATE NEXT STEP: Get feedback on what indicators to track and report on. Online Survey • IMPORTANT: Public Health and the Medical Community leadership on improving health in our communities

Community Health Needs AssessmentMoving towards a Healthy North Central Washington NCW is one regional family…everyone has a role to play • Okanogan County CHNA Steering Committee in development • Okanogan Public Health and Mid Valley Hospital are leading the efforts • Other partners are coming onboard • Grant County is already in the process of completing a Community Health Needs Assessment and we look forward to collaborating in future efforts • With the right incentive in place, we can make NCW a healthier and thriving region
