Conclusions of Investigation • Mine operator had not provided newly employed, experienced miner training or task training • Contractor had not provided newly employed, experienced miner training or task training (Overlapping citations issued) • Newly employed, experienced miner training document (5000-23) indicated that training had been provided by an uncertified instructor • Proper equipment blocking techniques were not used • Coal truck driver was performing duties of a crane mechanic and was obviously inexperienced at task being performed

  8. Contractor Fatalities • 34 or 21.4% of the 159 coal fatalities were contractor fatalities • 24 or 70.5% of the contractor fatalities occurred in the Tri-State area • 11 or 32.3% of the contractor fatalities occurred in SWV
