  • Mine operator had not provided newly employed, experienced miner training or task training

  • Contractor had not provided newly employed, experienced miner training or task training (Overlapping citations issued)

  • Newly employed, experienced miner training document (5000-23) indicated that training had been provided by an uncertified instructor

  • Proper equipment blocking techniques were not used

  • Coal truck driver was performing duties of a crane mechanic and was obviously inexperienced at task being performed

Training link is being broken between training room and job site
