  3. INTRODUCTION • Majority of training will be in the Sports Medicine Center • Opportunity to rotate on other services related to sports medicine and orthopaedics • Time and adequate support services available for research activities related to Sports Medicine

  4. HISTORY • 1994 – Family Medicine/Sports Medicine Fellowship (ACGME approved 1995) • 2000 – PM&R Sports Medicine Program • 2002 – Orthopaedic Upper Extremity Fellowship • 2004 – PM&R Sports Medicine Fellowship

  5. CERTIFICATION • Accredited/ACGME PM&R Sports Medicine Fellowship (2009) • PASSOR approved • Open to board eligible graduates of ACGME accredited PM&R residency programs

  6. SPORTS COVERAGE • Divine Savior Holy Angels High School • Training room (Wednesday afternoons 4:00-6:00 PM) • Volleyball, basketball and soccer (home games)
