Introduction • About the Fort Drum Regional Health Planning Organization (FDRHPO) • FDRHPO is a not-for-profit agency created in 2006 to strengthen the health tri-county care system surrounding Fort Drum, NY., home to the 10th Mountain Division, the most deployed division in the US Army during the past 25 years and the only division level fort without an on base hospital.

Introduction • About the North Country Health Information Partnership (N-CHIP) • N-CHIP was formed in 2010 to provide community-based support services to healthcare professionals, improving the quality of, and access to, regional health care by leveraging health information technology (HIT). • Our cost effective support model ensures that participating providers meet current/future HIT requirements. • We aim to achieve quality, access and economic benefits as envisioned in the CMS Triple Aim while supporting sustainable medical practices

Provider and Key Stakeholder Engagement • The Provider Executive Committee • The Provider Executive Committee (PEC) was formed at the very beginning of a regional EHR/HIE project and has been the clinical governance group ever since. • The providers determine the “what” based on the provider, patient and/or care system needs and the workgroups/staff and regional resources determine/implement the “how”. • This ensures we have the cart and horse in the right order. • The future…

Provider and Key Stakeholder Engagement • The Details or “How Do We Do This?” • Conduct a stakeholder analysis to ensure appropriate representation in the governance process before any ideas get developed • Give them budget control/approval and use as a prioritization venue • Part of their role is advocacy, if they are not managing up the initiative(s), you are losing a huge opportunity • The organizations they are representing have to give them the authority to speak on their behalf and reconcile their own operational priorities with those of the region • The Chair – only a facilitator, no authority over the group • Feed them

  9. Introduction Mission: To provide leadership in securing comprehensive servicesacross the continuum of care through collaborations with our network partners. With help from our Members, Executive Director and Executive Board, we've established a set of goals and objectives designed to lead the constituents we serve in securing a continuum of care services throughout Lafayette County and surrounding areas. Additionally, these goals and objectives are the benchmark by which the region’s success, and ours, will be measured.

  10. Needs Assessment • Steps for success: • Gather input on the communities health challenges and barriers • 2. Identify solutions that substantially expand and strengthen access to health care and quality of health care services—while considering programs already underway • 3. Look beyond the usual programs and players in the community to seek high-impact approaches to achieve substantial changes in access and quality of health care services • 4. Create a knowledge base that is shared both locally and with other county teams involved in this initiative • 5. Develop evidence-based, high-impact strategies along with a work plan for implementation with a strong evaluation plan.

  11. Implementation Implementing strategies that have the capacity to make dramatic improvements to quality health care delivery and access for our region’s underserved is the goal of HCC. We formulated a committee of leaders and stakeholders to support these programs. Individuals working in the following sectors were encouraged to participate: Migrant farm worker staff, dental, social service, government, education, agriculture, senior services, faith-based and business. Video: Collaboration is Key Building resources (RELATIONSHIPS) will build sustainability.
