  2. What is the Special Kids Network System of Care? The Special Kids Network System of Care (SKN SOC), in partnership with the PA Elks Home Service Program, helps children and youth with special health care needs by helping their families get the services and support these young people require in order to thrive in the community and develop to their full potential. The SKN SOC SKN SOC History: • Continued funding through the Federal Title V Maternal and Child Health Services Block Grant since inception in 1996 • Has focused on children with special health care needs through age 21 • Initially began as an information and referral service SKN System of Care: 1-800-986-4550 Website: www.gotoskn.state.pa.us

  5. A New Initiative Two significant changes were introduced: • Parents of children with special health care needs hired as Regional Coordinators • A partnership with the PA Elks Home Service Program provides service coordination SKN System of Care: 1-800-986-4550 Website: www.gotoskn.state.pa.us

  7. PA ELKS HOME SERVICE PROGRAM The Elks Home Service Program: • offers free in-home care coordination services; • provides services to persons of any age with a disability that began before age 22; and, • services can continue throughout a person’s lifetime, if needed.

  8. PA ELKS HOME SERVICE PROGRAM The Elks Home Service Program: • provides home visits by a nurse or social worker; • offers one-on-one help to navigate the system; and, • referrals can come from any source. www.paelkshomeservice.org

  10. The SKN SOC • The SKN SOC offers: • Service coordination through a partnership with the PA Elks Home Service Program; • it starts with a call to the SKN SOC Helpline 1-800-986-4550 • as needed, caller referred to Elks Nurse • Elks Nurse contacts family to begin service coordination

The SKN SOC The SKN SOC offers: • Regional Coordinators working in communities; • to enhance access and availability of services • to ensure families are partners in decision making at all levels • to facilitate opportunities for families to identify issues of concern to them.
