Healthy Living Team • Healthy Living Team is part of Central Essex Community Services and mainly focussed on Health Promotion/Prevention • Examples of projects we run: • Exercise referral • MEND and mini MEND • Cooking Classes • Brief Intervention Training • Weight management Programmes • Falls Prevention • Health Trainers

  3. Role of Health Trainers • Relative new role within the NHS • No health experts! • Motivators • Sign-posters • Neighbour approach • Non judgemental

  4. Health Trainer Project in Mid Essex Structure Employed Health Trainers Volunteer Health Trainers 1:1 Health Trainers (3.9 wte) Youth Health Trainers Community Health Trainers Prison Health Trainers

The 1:1 process Step 1 • Referral: Self or Social Prescribing Pad Step 2 • Client will receive invitation for first assessment to assess if client is eligible Step 3 • Client and HT work together to improve lifestyle • Work with behaviour techniques to change lifestyle • Average of 16 hours a client • Different types of interactions; face to face, supporting client to activity, email, text, phone call • Review progress Step 4 • Sign off • 3 outcomes: successful, partly successful or no success • If outcome is not successful; always leave the door open. Step 5 • Maintenance check, Check in several months to asses progress of client.

Suitable referrals 1:1 • Over 16 years old • Increase in levels of Physical Activity • Improve mental well-being • Weight and lifestyle management • Change to a healthier way of eating • Giving up smoking • Raise confidence/self esteem • Support the implementation of professionals’ advice eg following a diet sheet from a dietician • People who are willing to change

  7. Volunteer Health Trainers • Providing basic health information to people who want information • Signposting to the 1:1 Health Trainer or other relevant services • Supporting and organizing activities to improve health (cooking courses etc) • Assessing needs

  8. Data Collection • 1:1Health Trainer completes detailed lifestyle paper work on each client • National Database ‘Trainersinhealth’ • SystmOne in future • Volunteer use activity sheets

  9. The Benefits • Bridge between primary care and the voluntary sector. • Knowing what options are available to offer • May reduce visits into primary care • Can support long term conditions • Lead to a healthier community

Some figures • Start up year 2008 • 157 clients • 150 assessments • Issues being addressed: diet, exercise, self esteem • 40% has already completed health plan (others are still in process) • Improvements in self efficacy, health and WHO well being measures • 40 Trained Community, Prison and Youth Volunteer Health Trainers • Targets 2009/2010 • Minimum of 300 clients and 50% success rate • 60% clients form MSOA’s • 40 Volunteer Health Trainers
