  5. Approach • Core Problem • Medicare Requires: Quality Documentation from Qualified Provider • Typical ordering provider doesn’t/won’t/can’t create this quality documentation.

  6. Approach • Relationship problem • Establish relationship with ‘Functional Level Assessors’ (Doctor, Physical Therapist, Physiatrist, PA, LNP, Clinical Nurse Specialist) • 2 referrals • Explain need for enhanced documentation • Explain how to complete required documentation

Approach • Technology problem • Use ‘new’ workflow and clinical relationship in manual setting. • Bring in technology ‘low hanging fruit’ • 3rd party HIH for connection to esMD Gateway • Consider a portal proof of concept for document exchange between concerned parties. • Beyond Scope: Provider Directory, Digital Signatures, Direct integration with Functional Level Assessor EMR

  8. Goals (Primary) • Demonstrate usage of LLP eClinical Template in manual workflow • Effectively find and coordinate with functional level assessors? • Documentation meets CMS requirements? • Bundle documents as PDF and communicate with esMD Gateway

  9. Goals (Secondary) • Create a portal proof of concept to facilate exchange of documents between functional level assessor and prosthetist. • Create a “closed network” system where all actors are known and validated through appropriate mechanism
