Yolo County CNAP: History • There was a Yolo County CNAP group until 2008 • In 2012 the Health Department revived the CNAP process • Led by Amanda Nelson and Laurie Green • The process began by gathering contact information of FNS funded partners and sending an email invitation in January 2012

Yolo County CNAP: Partners 2012 partners who consistently attended meetings: • Health Dept.- WIC and Nutrition Network • First 5 Yolo • CalFresh • UC Cooperative Extension Nutrition Education Program- Nutrition Network • Yolo County Food Bank • Yolo County Children’s Alliance • California Dept. of Education- Nutrition Services • School Foodservice Directors • Farm to School Yolo Task Force • Elderly Nutrition Program • Office of Education- Head Start

Yolo County CNAP: Meetings • First meeting in February 2012 • Meetings in March, April, June, July, Sept, Oct • Prepared by listening to CCLDHN CNAP webinar, looking at other CNAPS and formulating a goal/plan to present • Focus of first two meetings: • Agencies sharing to get to know one another • Updates from agencies • Coordination about how we could help each other

  5. Yolo County CNAP: Vision & Goal Vision: All Yolo County residents enjoy healthy foods/beverages and physical activity every day. Tagline: Yolo thinks healthy first! Goal: Develop and support collaborative efforts and interventions to increase the number of Yolo County residents that: • participate in nutrition programs, • engage in active living, and • follow the current Dietary Guidelines for Americans.

Yolo County CNAP: Action Plan How we created the action plan: • Invited in a facilitator, Melody Steeples from CAN-Act, to lead two brainstorming meetings and generate ideas for action plan • Multiple drafts of the document were sent to members and feedback incorporated • Current document is posted on CCLHDN’s website

Yolo County CNAP: Lessons &Successes Lessons Learned/Tips • Build CNAP action items from existing efforts in the County • Use other County’s CNAPs for guidance • Have set meeting dates and times for consistency • Talk about the CNAP at other meetings you attend- you might find more interested partners Successes • Good meeting participation and input from members • Assistance from outside facilitator helped envision successful CNAP and generate ideas • Improved communication/coordination among Network grantees • Opportunity to network and build relationships with other nutrition professionals in the County
