  2. DOT Drivers Need TWO Good Eyes

  3. Distant Visual Acuity: At least 20/40 (Snellen) in each eye (corrected or uncorrected). At least 20/40 (Snellen) distant binocular acuity (corrected or uncorrected).

  4. Peripheral Field of Vision: At least 70 degrees in the horizontal meridian in each eye.

  5. Color Vision: Must be able to recognize colors of traffic signals and devices showing: Red Green Amber

  6. Monocular Vision Exemption: Still do exam and determine if “otherwise qualified” and mark vision exemption. Max 1 year certification At annual recertification, driver should present valid exemption and copy of the specialist eye examination report. Must have annual medical exam and annual eye exam by specialist.

  7. VISION EXAM CAN BE PERFORMED BY: Medical Examiner Ophthalmologist (SIGN PG. 2) Optometrist (SIGN PG. 2)

  8. “Grandfathered” Vision Exemption: Very few drivers qualified by operation of 49 CFR 391.64 Must have annual medical exam/annual eye exam by specialist Must be “otherwise qualified” Must have documentation of participation in the study Max certification is 1 year Mark “Qualified by operation of 49 CFR 391.64”

  9. Consider the following: • Larger blind spots • Longer turning radiuses • Increased stopping times

  10. Snellen Eye Chart Versus Titmus Vision Tester • If Snellen: • Accurately Measure And Mark 20 Feet • Well Illuminated With A White Light • Refer And Hold Card If Question On Vision

  11. Physical Exam: • Pupillary equality • Reaction to light and accommodation • Ocular Motility • Ocular muscle imbalance • Extraocular movements • Nystagmus • Exophthalmos

  12. Remember to review eye health history, question, examine, and refer if concerned.

  13. Mark “corrective lenses” on Page 3 and card and advise the driver to wear them at all times while driving and bring a back up pair. I recommend giving the driver a letter stating above.

  15. CMV drivers only need ONE good ear

  16. Types of tests for hearing: Forced whisper Audiometric testing

  17. With or Without Hearing Aids May need to go to audiologist or ophthalmologist for accurate hearing tests. Mark “wearing hearing aid” on page 3

  18. Forced Whisper: Demonstrate? Perceives at not less than 5 feet with or without hearing aids in the better ear.

  19. Audiometric Testing: Must have an average hearing loss, in one ear, less than or equal to 40 decibels (dB). Only average the test results for 500, 1,000, and 2,000 Hz. Calibrated to American National Standards Institute (ANSI) ONE GOOD EAR. With or without hearing aids (may need referral).

  20. Review ear history and EXAMINE for:Scarring of the tympanic membranesOcclusion of the external ear canalPerforated eardrumsBalanceOther signs of disease that can affect hearing/balance

  21. Cerumen Impaction: If hearing ability seems to be affected by cerumen impaction, refer the driver to his PCP or local walk in care for debridement/lavage. The driver should wait 24 hours after procedure before returning for hearing evaluation.

  22. Guidance/Advisory Criteria: Meniere’s Disease: The Conference on Neurological Disorders and Commercial Drivers report recommends to disqualify. Vertigo: Uncontrolled vertigo is disqualifying. The Conference on Neurological Disorders and Commercial Drivers report recommends that the driver may be certified after completing at least 2 months symptoms free for: Benign Positional Vertigo Acute and chronic peripheral vestibulopathy Labyrinthine Fistula: The Conference on Neurological Disorders and Commercial Drivers report recommends to disqualify. Nonfunctioning Labyrinth: The Conference on Neurological Disorders and Commercial Drivers report recommends to disqualify.
