Project Order Process

1. The Nation of Islam Ministry of Agriculture NOIMOA Bulk Staple Goods Purchase Project 3 Project Order Process Informparticipants who have completed the survey that we are now collecting money orders for their purchase total amount by Sunday May 20, 2012. Money Orders should be completed like this Verify their order information and check the total amount a) sign top form, tear off the bottom and use it to enter info on the Live Data Spreadsheet 3. Collect Money Orders and distribute receipts. Buy a three layer purchase order receipt book (1st layer for you, 2nd layer for Mosque or Study Group Secretary, 3rd layer for purchaser) Collect money orders and issue a receipt for orders (be certain to sign your name as receiver of funds) Verify that purchaser’s name and complete address is filled in properly. Be sure that money order reads as in the illustration above For new people who are interested, present the NOIMOA Staple Goods Order Form (see form attached) Have them to complete form and return it with money order ONLY by Sunday May 20, 2011 3Year Economic Savings Program Participant’s Name 4855 S. Enter or update all order information on your Live Data Spreadsheet • a) Print out a copy for mailing to Chicago along with money orders • 5. Prepare documentation to be sent to The National Center • Package should include the following: • Each individual money order • A copy of your Live Data Spreadsheet • DO NOT WRITE IN ANY ORDERS ON THE SPREADSHEET. • TO RECEIVE GOODS, ALL ORDERS MUST BE ON THE LIVE DATA SPREADSHEET. • WE USE THAT TO PLACE THE ORDERS. • 6.Mail money orders and receipts to 3 Year Economic Program by May 21, 2011 • Place the above documents in a large envelope and mail to : • 3 Year Economic Program Savings Program • c/o Staple Goods • 4855 S. Woodlawn • Chicago, IL 60615 • Ask for a tracking numberand forward the number in an e-mail along with your name and the name of your MOA city to support@noimoa.com • 7.Please act in a timely manner. We cannot make the order as a group until everyone has paid. • 8. Tentative ordering schedule (subject to change) • May 24, 2012 All money orders should be received in Chicago • May 28, 2012Bulk purchases will be made • June 8, 2012Product arrives at central distribution center • June 11, 2012Delivery to City location begins • 9. You will be notified of your delivery location where you can then make arrangements to go and pick up your goods
