Terminology • Release- when the Dashboards and Reports are updated to include new data • Other enhancements are also included • Enhancement- when the format of Dashboards and Reports are changed • May include new tables/charts, column moves, label name changes, etc.

June 2013 Changes All-Payer Patient List Changes Added two new fields • NEW_PATIENT_PO_FLAG • indicates if a patient is new on the current month’s patient list compared to the previous month’s list for the PO • NEW_PATIENT_PU_FLAG • Indicates if a patient is new on the current month’s patient list compared to the previous month’s list for the Practice These fields are included at the end of the records (prior to the Payer pass-through fields)

June 2013 Changes All-Payer Patient List Changes Dropped Patients List • Includes a list of patients who appeared on the previous month’s list, but do not appear on the current month’s list. • This file includes patients for the entire PO. (There are no practice-specific dropped patient lists.) • The record layout of the dropped patient list is the same as the other patient list files.

July 2013 Changes All-Payer Patient List Changes Layout Changes The lists will include fields in the following groups/order: • Demographic fields: Payer Specific Member ID, Member First Name, Member Last Name, Member Date of Birth, Age, Gender • Attribution fields: Attributed Physician NPI, Attributed Physician First Name, Attributed Physician Last Name, Attributed Practice Unit ID, Attributed Practice Unit Name • Risk fields: Concurrent Risk Score, Concurrent Risk Group, Prospective Risk Score • Cost: Standard Cost • Utilization fields: # of Inpatient Visits,# of ED visits, # of Readmissions, # of visits to any PCP, Most recent PCP visit date, # of Chronic Conditions, # of maintenance drug prescriptions • Chronic Condition flags: Diabetes, Asthma, CHF, COPD, HTN, Obesity, ADHD, CAD, CKD • New to PO/PU fields • Payer-specific pass-through fields
