  2. What is SCR? • SCR • Summary Care Record • What patient have always thought exists • Not part of the GP held record • Summary of: • Current medication • Adverse Reactions • Allergies • Discontinued meds

  3. Who will benefit? • Patients • when in Unscheduled Care settings • Clinicians • Walk in Centre • Out of Hours • A&E • Ambulance • GP’s with Temporary patients (new move in’s, students)

  4. What are the Benefits? • Increased patient safety • Immediate availability of patient details • More appropriate treatment at point of assessment • Cost savings • repeat prescriptions • duplicated tests • appropriate treatments • Can contain violent patient indicators

  5. When will the SCR be created? • Following these activities: • Mail out to all patients • 31st March • Patients over 15 ¾ years • Opt out period – consent model see next slide • Data Quality & Paper Light Accreditation • Consultation with practices • Waves – phased implementation • PDS synchronisation – phone #

  6. Creating the Record:Do you want a Summary Care Record? Do nothing and a record will be created for you. Inform your GP Practice of your choice and no record will be created. NO YES

What does it mean to GP Practices? • PIP – who will patients contact for answers? • NHS Direct • PALS • Practices • Preparation is everything!! • IT Training will provide relevant awareness training for all patient facing staff in your practice • Very tight timeline – every practice must be done by 30th March in time for mail out on 31st March

Processes in Practice • Opt out Read code 93c3. • Opt out form to patient • Safe record keeping for returned forms • Any previous opt outs? • Pull of list to check if patient has re-confirmed • New Patient registrations process • To include with new patient packs • FP69 returned mail process • directly returned to the practice? • Returned to FHS?

  9. SCR on-going • On going data quality requirement following initial 3 years continuity • Continuous update to SCR via smartcard • Smartcard security becomes very high profile • RBAC access levels • Enrichments • Joined up systems – see benefits
