  1. Developing programs • What is your program presently doing? • What needs are not being met? • What are the priorities for you program? • Have you completed a needs assessment? • Where do you want your program to be in the next couple of years!

  2. What do I do first. • Sit down with you staff, even if it is only you and the cook, you need to make a plan. • You may even include the Advisory Committee/Site Committee. • Know what resources are in your area.

  3. Maintaining information • Each elderly participant should have a file. • All documents are kept confidential. • Records with additional information can help when you are looking for services, such as income, social security numbers, etc. • Assure the elder about the confidentiality rule, everything is kept locked.

  4. Contact local providers • Make a list of services as well as phone numbers, 800 numbers and e-mail addresses, etc. • Contact people are helpful, especially to bypass a computerized phone system. • What information is needed to apply for services, such as check stubs, bills, etc.

  5. Distribute or post your directory • Have copies available for the elders. • Post your list in your office, senior center. • Share with other programs, to help refer the elderly to service providers.

  6. Don’t forget to document • Remember to document each referral, many times we forget we just provided a service and didn’t write it down. • Have a system to track all referrals. • The list from the Title VI training manual helps to track the services provided to the elderly.

  8. Maintain confidentiality • Client information must remain secure. • If working with other agencies, make sure to have permission slips signed for release of information.
