What’s the purpose of this presentation? • To ‘Promote and Cascade’ the message of Denbighshire’s Health, Social Care & Well-Being Strategy (HSCWB) 2008/11 ensuring that we: Raise awareness of the Health, Social Care & Well-Being Strategy and the Partnership Develop ownership of the Strategy from the very beginning • Provide meaningful engagement throughout its development

  5. What is the HSCWB Strategy? • The HSCWB Strategy is now one of the 4 major Strategies/Plans in Denbighshire. It must provide a co-ordinated response by inter-linking and cross-referencing with other major local plans and compliment the development of the Community Services Framework (folder attached)

What is Denbighshire’s HSCWB Strategy? • This is the second Health, Social Care & Well-Being Strategy – the first covered 2005-2008 • It is a statutory duty that each Local Health Board (LHB) and Local Authority (LA) jointly formulate and implement a strategy for the health and well-being of its local population • It will provide us with the opportunity to further strengthen collaboration and integration between all local partners, both statutory and non-statutory

  7. What is Denbighshire’s HSCWB Strategy? (cont) • Main requirement for the second round of the Health, Social Care & Well-being Strategies is that in each area the agreed Strategy: • Presents a consolidated vision of what the local partnership expect success to look like in March 2011 • The HSCWB Strategy will be the main driver for commissioning health improvement and health and social care services!

What is Denbighshire’s HSCWB Partnership? • Denbighshire’s Health, Social Care & Well-Being Partnership is comprised of partners from statutory and non-statutory services ie. local authority, LHB, the Trust, NPHS, voluntary sector and independent sector. • A Lead Officer Group (LOG) and Strategic Partnership Board (SPB) made up of key representatives from across the partnership direct the development of the HSCWB Strategy and the work of the HSCWB Partnership

  9. How is Denbighshire’s HSCWB Strategy developed? • Denbighshire Needs Assessment was completed in March 2007. The report tells us: • where we live and how healthy we are • what has been done and what is being done to improve our lives whilst we are young and as we get older and as the communities we live in change • The information gathered will help us to decide how to plan for better futures for everyone in Denbighshire • The Needs Assessment will form the foundation of the Strategy • This needs assessment is an on-going process, not a one-off exercise

Developments… (cont)HSCWB Strategy 2008-11 • 8 Themes have been agreed to achieve our vision • The Themes were agreed by partners involved in workshops, engagement activities etc… • Each Theme will contain 10 actions and each service user group will have a minimum of 1 action under each of the Themes • The Strategy will identify underpinning principles including equalities, public involvement, inclusion, independence • Each theme will be followed by an agreed ‘Meaning’

  11. Agreed Themes • Tackling Poverty and Deprivation • Environment • Positive Lifestyles • Early Intervention & Crisis Support • Changing Needs/Responsive Services • Longer Term Needs • Protecting & Safeguarding • Right Services in the Right Place

  12. What Difference will the HSCWB Strategy Make? The aims of the HSWB Strategy are: • To improve health, social care & well-being for the people of Denbighshire by: • Addressing the full range of issues that affect people’s health, social care & well-being • Improving the provision, quality and delivery of and access to more integrated NHS, local government, voluntary sector and private sector services and other functions which affect health, social care & well-being; recognising the need to develop bilingual services • Establishing a stronger basis for planning, investment, commissioning and performance review at a local level; and • Promoting joint delivery of the vision

  13. What Difference has the HSCWB Strategy Made? • There have been so many positive outcomes since the development of the first Strategy. Therefore a Health, Social Care & Well-being Achievements Report has been created which attempts to capture and demonstrate the achievements to date. • In addition there have been some fantastic developments with regard to working together with our partners to design and deliver services around the needs of users. These innovative joint project developments funded through Joint Working Grant (JWG) funds have been illustrated in an easy to read leaflet • Both the Achievements Report and JWG leaflet are available in attached folders or accessed on our newly developed Health, Social Care & Well-Being Partnership website at: www.healthydenbighshire.co.uk

  14. Where do you fit in? • The HSCWB Strategy’s underpinning principles include equality, public involvement, inclusion and independence. Each of these principles has a direct impact on our lives. • This is your chance to help shape the next Health, Social Care & Well-being Strategy. You really can make a difference!

  15. Why should you get involved? • This is a real opportunity to be involved in influencing the future of health and social care services within Denbighshire • We want to hear your views to take them into consideration!

  16. How can you get involved? • There is a planned consultation phase from October to December 2007. A number of events, publications etc. have been developed. You or your organisation are more than welcome to be represented at any of these events etc. We would also like to be made aware of any planned events, press releases, meetings, conferences, workshops, newsletters etc. so that we can ‘piggyback’ on them to ensure effective and countywide engagement. • We welcome your thoughts, opinions and ideas on the planned methods of consultation. • Check out our newly developed HSCWB Partnership website healthydenbighshire.co.uk for upcoming events and for the opportunity to feedback on-line

The VISION Denbighshire will by 2020 seek to become an enjoyable place to live and work – a County which is widely recognised as using its natural attractions to enhance the health and well-being of residents and visitors; as listening to Denbighshire people and innovative in meeting the health and social care needs of Denbighshire's diverse communities
