  1. 血漿中微量營養素、抗氧化酵素基因多形性及砷代謝能力與泌尿道上皮癌的相關性研究血漿中微量營養素、抗氧化酵素基因多形性及砷代謝能力與泌尿道上皮癌的相關性研究 • 探討抗氧化酵素含錳超氧歧化? (MnSOD)與觸? (Catalase) 基因多形性及血漿中微量營養素,砷代謝能力、香菸及測量尿液中氧化傷害產物(8-OHdG)與泌尿道上皮癌危險性的相關性。本病例對照研究於臺大醫院泌尿科門診所收泌尿道上皮癌病患152位,對照組為臺大醫院泌尿科、臺北醫學大學附設醫院及臺北市立萬芳醫院健檢民眾,排除泌尿道癌症者,經年齡性別配對之對照組共收案158位。取得研究對象同意書後以標準化問卷訪視研究對象,收集人口學基本資料、生活習慣、職業暴露及個人與家族疾病史並收集其血液與尿液檢體。利用高效能液相層析儀串聯氫化式原子吸收光譜儀測定尿液三價砷、五價砷、單甲基砷酸與雙甲基砷酸濃度。萃取血液DNA,利用聚合酵素連鎖反應(polymorphism chain reaction; PCR)增幅所需片段。再利用限制酵素片段長度基因多形性(RFLP)分析含錳超氧歧化?與觸?的基因型。以高效能液相層析儀分析血漿中微量營養素含量。結果發現,疾病組總砷、無機砷及單甲基砷酸百分比顯著高於對照組,雙甲基砷酸百分比與二級甲基化指標顯著低於對照組,顯示疾病組砷甲基化代謝能力比對照組差。維生素E濃度越高泌尿道上皮癌的危險性顯著的降低。維生素E濃度越低且二級甲基化指標越低使得泌尿道上皮癌顯著增加。雙甲基砷酸百分比越高且曾經抽菸者泌尿道上皮癌顯著增加。而在含錳超氧歧化?基因型與觸?基因型未與疾病有顯著關係。顯示砷代謝能力差、維生素E濃度較低及抽菸是泌尿道上皮癌的重要危險因子。以總砷為例,若同時具備總砷濃度較高、維生素E濃度較低及曾經抽菸此三種危險因子比沒有任何一個危險因子泌尿道上皮癌危險對比值顯著增高至16.67 (95%信賴區間4.78-58.21)。
