Early Intervention Parent Support Plan • Mission of First Steps Georgia • Goals: To provide expectant parents and families with children ages birth to five 1) current information on topics of interest and relevance to the family; 2) information on community resources relevant to the family; 3) services to strengthen the protective factor of concrete connections to promote maternal and infant health for expectant parents and parents of children birth to five. • First Steps Georgia Summary

Planning and Implementation: With community partners, develop and implement a First Steps Georgia plan to provide universal, early intervention services and support for families: • Target Population/Service Delivery Location • Access • Identifying Needs/Community Resources • Documentation • Coordination • First Steps Georgia Summary

  6. Services and Support- Just as every family is different, every community is unique and the design of these plans for supporting families will be different. The proposal should be tailored to the specific needs of the community. At a minimum, the following functions (or services) are required: • Initial contact • Linkages to community resources • Educational materials relevant to families from each of the required topic areas • Mandated materials • Community resource guide • Parent telephone number • Contact information • Survey • First Steps Georgia Summary

Applicants must: • Be a public government entity or 501(c)(3) non-profit • Serve as the fiscal agent for the grant and the point of contact to GOCF or Provide a MOU with fiscal agent • Be able to submit timely and accurate program and financial reports • Be responsible, liable, and oversee financial, program and post-award reporting requirements • Plan or continue to provide universal, early intervention supports and services in accordance with the First Steps Georgia Plan. • Award Eligibility

Deadline for Proposal Submission: Proposal cannot be hand delivered and must be mailed via postmark by August 9, 2013 Governor’s Office for Children and Families Child Abuse and Neglect Prevention Program 7 Martin Luther King Jr. Drive, Suite 351 Atlanta, Georgia 30334 • Grant Award Period: 12 months, October 1, 2013-September 30, 2014 • All service delivery activities (new, enhanced or expanded services) must begin within 90 days of the start date of grant award. • Proposal Submission

  9. An eligible applicant must be applying to implement or continue universal, early intervention support and services in accordance with the First Steps Georgia plan. GOCF requires that funded programs will comply with performance criteria and fully participate in the following evaluation and report: • Performance Criteria (Page 5 (A)) • Program Eligibility

  10. GOCF Grants Management System: • Georgia Home Visiting Information System (GEOHVIS) • Central Intake Data System (CIDS) • Satisfaction Survey • Program Eligibility

  11. Each applicant is eligible to apply for a 12 month “mini-grant” for services and supports that meet the criteria of First Steps Georgia and is a start-up or continuation of an existing First Steps grant award. • Applicant may apply for funding in the amount of $6,000 or $12,000 conditional upon the project number of additional families that will be served annually: • $6,000- 100 additional families • $12,000-200 additional families • Grant award period: October 1, 2013-September 30,2014 • Grant Award Amounts

  12. The granting of funds in one year is not a commitment to continue assistance in the future and does not guarantee future funding. If there is continuation funding, it will be contingent upon: • Organizational capacity; • Performance history and grant compliance; and • Availability of GOCF funds • Continuation Funding

One (1) original application including application narrative, application budget request and application attachments • All application face sheet are signed in blue ink • Four (4) additional copies • One(1) electronic copy, CD or Thumb Drive • Bind the original application and copies with binder clips • All documents listed on page 7 • Submission Date: On or before August 9, 2013 • Proposal Submission Instructions

Applicant is requesting funds to support First Steps in a county where First Steps is currently receiving funds through a Maternal, Infant and Early Childhood Home Visiting (MIECHV) grant; • Submission fails to be postmarked by U.S Postal Service or commercial carrier by deadline date; • Submission fails to include the original signature of the executive officer of the fiscal agent (applicant) on the application face sheet; • Submission fails to include the application program narrative, application budget and application attachments; • Application lacks original signatures, where requested. • Disqualification Factors

  17. The narrative is a detailed statement of the work to be undertaken and answers who, what, when, where, why, and how statements about the grant proposal. • Restate and number each question followed by a response • 9 page limit • Proposal Narrative

  18. One page limit • Clear, concise and complete • Statement of need • Description of how need relates • Description of target population • Description of chosen geographic area • Description of the array of services and supports • Proposal Summary

  19. 1/2 page limit • Describe implementing agencies • Does the fiscal agent have the ability to maintain a quarterly cash flow? • Discuss the applicant’s qualification and experience managing grants. • What is the agency’s history, qualifications and experience in managing First Steps? • Administration

  20. 1 1/2 page limit • Provide specific local data such as demographics, poverty rate, child abuse, teen birth rate, etc. • Describe why the program is important and how it will impact family outcomes such as child maltreatment prevention, health and school readiness outcomes. • Describe the target population to be served by First Steps • Describe the location of First Steps services • How many individuals will be served during the grant award period by First Steps? • Target Population

  22. 4 pages • Describe in detail the program design, format and the methodology • Status- What is the current number of families served? Is the program planning to serve 100 or 200 additional participants? • Methods and Procedures

The screening process for First Steps Georgia (once families have been referred to this service) must be the Great Start Georgia Core Screening Tool. • Program Entry: Describe process for program entry into the First Steps in your community by providing a detailed response to each of the following: • 1. Target populations and service delivery location(s) • 2. How will the program ensure parents of all ages and all ethnic and socio-economic backgrounds have access to First Steps • 3. Describe the screening process for families • 4. Describe the referral process to community partners that will provide supports and services to families • 5. Describe the array of supports and services that will be provided to families by your community partners. • Methods and Procedures

  24. Initial Contact: Describe First Steps initial family contact in your community by providing a detailed response to each of the following: • How will the initial contact be made • Relevant and age-appropriate materials and information • Describe the localized, annually updated Community Resource Guide • How will the program provide contact information to reconnect with the First Steps staff? • Provide a description of how the program will provide families with a parent support telephone number, available 24/7 if possible. • Methods and Procedures

  30. Total Wages Earned Monthly • This should reflect a person’s annual salary divided by 12 months. • If the employee is hourly, multiply his or her hourly rate by the number of hours they are anticipated to work in a year and divide that number by 12 months. • $8 *2080 hours (52 weeks*40/hr per week) 12 • Personal Services

Taxes and Benefits • This should be the amount of taxes and benefits the employer pays on behalf of the employee. • If your agency uses a fringe rate, multiply this by the employee’s annual salary and divide by 11. • DO NOT include the amount of taxes that are removed from an employee’s paycheck, these are already accounted for in his or her salary. • Personal Services

  32. % FTE • This is the amount of time an employee spends working on the GOCF project. • Example: Sally works 30 hours per week for the school system. If on average she spend 15 hours per week on the GOCF program, her FTE should be (.5). If on average she spends 30 hours per week on the GOCF program, her FTE should be (1). • Personal Services

  33. Number of Months • This should reflect the number of months an employee will work on a project. • If he or she only works 10 months out of the year on the GOCF project, then this is the number of months that should be applied to the grant. • Personal Services

  34. The Total Request should be calculated by using the following formula: ((Monthly Wage + Taxes & Benefits)*FTE)*Months • Personal Services

Narrative • All costs will need to be accompanied by a written description within the narrative • Example: • Personnel: • Personnel working directly with families – 20% (D) • Supplies • $3,000: First Steps Educational Materials • Trainings • GOCF /First Steps Trainings - $1,226 • Personal mileage: 300 miles per roundtrip *$0.565 = $195 *4 = $780 • Hotel: $95/night *4 nights = $380 • Per diem: 4 days at $ 16.50/day = $66 • Required First Steps Trainings • FY14 RFP-Financials
