  6. Why are referrals slowing for my county’s THP-Plus program?

  7. # of youth who “age out” annually: Jan. to Dec. AB12 Implementation Why are referrals slowing for my THP-Plus program?

Participants are older: 21-24 year olds (48%, up from 31%) • More participants are custodial mothers(40%, up from 32%) • More youth are entering from homelessness (25%, up from 18%) • Fewer youth are entering directly from foster care (14%, down from 31%) How has this reduction in youth who “age out” affected THP-Plus?

  9. In 2013: Statewide How many THP-Plus eligible youth are NOT being served?

In the face of a 40% reduction in youth “aging out” of foster care, THP-Plus providers must adopt new, creative outreach strategies. • These strategies will help maintain your housing census until January 1, 2015, when the number of youth who “age out” increases. How has this reduction in youth who “age out” affected THP-Plus?

Merced County Aspiranet:staying at capacity although experiencing significant drop in referrals, how? • Nurtures relationship with county • County provides list of emancipated youth so Aspiranet can conduct targeted outreach • County keeps Aspiranet updated on planned dates of discharge for youth turning 21 next year • Strong connection with ILP • Aspiranet fosters “buy-in” with ILP staff to encourage THP-Plus referral • Maintains close contact and reciprocal relationship • Accepts out of county youth • Uses word-of-mouth • Siblings, friends of THP-Plus participants Luis Madrid Aspiranet
