  2. COAL MINE FATALITY #1 - On Friday, February 6, 2009, a 70 year old contract truck driver with eight weeks experience on this job was fatally injured while driving a truck on a coal haulage road. The truck struck the left embankment at an angle, traveled approximately 97 feet down grade, and turned over on the road. The victim was fatally injured when he was trapped under the cab of the truck.

  3. COAL MINE FATALITY #2 - On Tuesday, February 17, 2009, a 27-year old tractor-trailer driver was fatally injured when he was struck by falling lumber. The victim was an over the road truck driver who was delivering a load of lumber to the mine on a flat bed tractor-trailer. A mine employee was using a forklift to unload the lumber from the trailer. The operator of the forklift attempted to remove two bundles of lumber from the driver's side of the trailer. When he raised the forks, two bundles of lumber fell off the trailer on the passenger's side and struck the victim.

  4. COAL MINE FATALITY #3 - On Thursday, February 26, 2009, a 50-year old contractor with no mining experience was fatally injured when he fell from a step ladder. The victim was standing on one of the steps of a 10 foot step ladder while assisting a co-worker with the installation of a rolling steel overhead door. The two contractors were positioned on either side the door when it began to roll from the fully opened position. The door struck the ladder, causing the victim to fall, striking his head on the concrete pad. He was wearing a harness, but the lanyard was not anchored.   

  5. COAL MINE FATALITY #4 - On Friday, March 6, 2009, a dragline oiler with 5 years of mining experience, was fatally injured at a surface lignite mine. While the dragline was being deadheaded or walked to a new cut in the pit, the oiler needed to add extra grease to the cam slide area of the walking mechanism. He walked from one shoe to the other through the dragline house, and was last seen standing on the end of the catwalk on the shoe. The victim was found in a pinch point between the shoe and the dragline house, approximately 10 feet from where he was last seen.

  6. COAL MINE FATALITY #5 - On Thursday, April 2, 2009, a 29-year old surface rock truck driver was fatally injured when he was pinned between the railing outside of the operator's cab and the dump bed of the truck he was operating. The driver had seen oil spillage on the ground during a run into the pit staging area. He exited the operator's compartment and was standing against the hand rail at the rear of the operators cab (drivers side) when the bed dropped.

  7. COAL MINE FATALITY #6 - On May 21, 2009, a 32-year old foreman/truck driver with 15 years of experience was fatally injured when the loaded rock truck he was operating traveled over the edge of a dump site. The truck rolled over, landed on its top at the toe of an approximate 60-feet high slope, and crushed the cab.

  8. COAL MINE FATALITY #7 - On Tuesday, June 9, 2009, at approximately 7:15 p.m., a fatal powered haulage accident occurred, resulting in the death of a 58 year old Section Foreman. A scoop operator was pushing/positioning a trailer loaded with approximately 220 concrete blocks, when the trailer became unattached from the scoop and rolled down grade, pinning the victim against an outby bridge conveyor.                 

  9. COAL MINE FATALITY #8 - At approximately 11:00 pm on June 23, 2009, a roof bolter was struck by a piece of shale that fell from the mine roof. The victim was in the process of setting a roof jack on the left inby corner of the crosscut between the No. 2 and No. 1 entries when he was struck by the rock. A portion of this corner had been mined, a normal practice at this mine. The rock measured approximately 4 feet long by 3 feet wide, and ranged from 9 to 11 inches thick. This section was mined with a full face continuous mining machine with integral bolting machines.

  11. METAL/NONMETAL MINE FATALITY #1 - On January 6, 2009, a 41 year-old laborer with 3 years of experience was injured at a sand and gravel operation. The victim was operating a skid steer loader underneath a belt conveyor that was being dismantled. Two coworkers were in an elevated manlift removing a 12-foot piece of 4-inch metal tubing from the leg supports of the belt conveyor frame. The tubing fell into the front of the skid steer loader as it approached the work area, striking the victim. He was hospitalized and died on January 9, 2009.

  12. METAL/NONMETAL MINE FATALITY #2 - On January 17, 2009, a 48 year-old mill operator with 22 weeks of experience was fatally injured at a crushed stone milling operation. The victim was loading material into a hopper with a front-end loader. He entered the hopper to dislodge frozen bridged material that would not feed onto the belt conveyor below. Coworkers found the victim engulfed in the hopper.

  13. METAL/NONMETAL MINE FATALITY #3 - On January 31, 2009, a 40 year-old contractor technician with 36 weeks of experience was fatally injured at an alumina milling operation. He was cleaning hydrate that had built up inside a 30-inch pipe in the plant. The victim was using a high pressure water hose when the unrestrained hose end and attachment blew out of the pipe. He was struck by the water.

  14. METAL/NONMETAL MINE FATALITY #4 - On April 7, 2009, a 36 - year old supervisor with 15 years of experience was fatally injured at a sand and gravel dredging operation. The victim was attempting to connect the 4160 volt cable for the dredge to load side terminals in the electrical panel when he came into contact with energized 4160 volt line side terminals.

  15. METAL/NONMETAL MINE FATALITY #5 - On February 19, 2009, a 61 year-old laborer with 12 years of experience was injured at a sand and gravel operation when he was struck by a crane's falling boom. The crane was lifting a crusher and the victim was acting as a signalman when the accident occurred. He was hospitalized and died on April 12, 2009, as a result of his injuries.

  16. METAL/NONMETAL MINE FATALITY #6 - On April 14, 2009, a 38 - year old contractor carpenter with 8 years of experience was fatally injured at a cement plant under construction. While dismantling a section of scaffolding, the victim lost his balance and fell when one end of the metal pan on which he was standing shifted unexpectedly. As he fell backward, the victim's safety lanyard slipped off the end of the horizontal scaffold to which he was tied.

  17. METAL/NONMETAL MINE FATALITY #7 - On April 21, 2009, a 51-year old contract laborer with 3 years of experience was fatally injured at a sand and gravel dredging operation. The victim was inside an excavation ditch while an excavator was maneuvering a concrete box into place. The chain used to attach the four leg sling from the box to the excavator broke. The box fell into the hole and struck the victim crushing him.

  18. METAL/NONMETAL MINE FATALITY #8 - On May 1, 2009, a 59-year old dredge operator with 3 years of experience was fatally injured at a dredging operation. The victim was trying to manually guide the wire cable onto one of two positioning winches on the dredge when he became entangled in it.

  19. METAL/NONMETAL MINE FATALITY #9 - On May 2, 2009, a 51-year old front-end loader operator with 8 weeks of experience was fatally injured at a surface clay operation. The victim parked a front-end loader on a loading ramp and was run over by the machine after he exited it.    

  20. METAL/NONMETAL MINE FATALITY #10 - On June 6, 2009, a 57-year old surface driller with 27 years of experience was fatally injured at a surface gold mine. The victim was drilling in a pit, exited the drill, and was walking in the drill area when he was struck by a flatbed truck as it backed up. The truck was in the area to collect drill cutting sample bags.

  21. METAL/NONMETAL MINE FATALITY #11 - On June 11, 2009, a 57-year old mechanic with 31 years of experience was fatally injured at an underground lead/zinc mine while checking for a hydraulic leak on a loader. The victim leaned into the ejector plate relief port in the back of the loader bucket. The ejector plate of the bucket was retracted, crushing him against the back of the bucket.

  22. METAL/NONMETAL MINE FATALITY #12 - On June 20, 2009, a 52-year old equipment oiler/greaser with 24 years of experience was fatally injured at an underground salt mine. He was cleaning equipment when a large roof fall occurred.
