  1. Work-Related Injury Procedure Injured EE (1) EE Health Dept. (501 Oak Ave) (1) Other HC Facility (2) The Steps: Workers’ Comp (1) EE seeks care at UC EE Health Dept or Other HC Facility. (2) (2) EE, EHC Dept or LAWR faxes form w/i 24 hrs** to WC. LAWR (3) (3) WC requires investigation by dept. LAWR: Investigation (4) LAWR investigates with EE supervisor & Safety Officer (SO involved if non-routine); faxes report to WC w/i 48 hrs. (4) Workers’ Comp (5) WC notifies EH&S if claim filed & information stored in db. ** The reporting time is 8 hrs for extremely serious injuries involving hospitalization or death (contact EH&S at 530-752-1493 ASAP). EE: employee SO: LAWR Safety Officer EH&S: Env. Health & Safety Unit (5) EH&S database
