Themes and Concerns • Ref: Journal of Adult Protection Sept 2004 • Sexual Abuse in Residential Settings • Sexual Abuse in Paid Domiciliary Care in residential settings • Independent living • 1/3 of all cases from indecent touching to rape are found in Residential Settings • Perpetrators are fellow residents, relatives, visitors and staff. The largest percentage of victims are female and the majority of perpetrators are male.

Themes and Concerns • Concerns cluster around a number of issues: capacity, sexual orientation, attitudes and policies, holistic assessment, recruitment and employment, vulnerable dependent adults, family relations and history, emotional aftercare, medical treatment and risk of STD. • With the amount of unsupervised work in homes, the issues of power differentials, sexual harassment, intimacy and boundary issues are also evident.

  4. Themes and Concerns • In the independent living setting, the elderly are subject increased vulnerability in their living space. Break and enter, theft, and sexual assaults are combined in this environment, with offenders between 18-35 and the victims 70-80 • Many of the offenders have long records and have themselves been subject to intra-familial and intergenerational abuse. • There are no long term data on these offenses or the impact on victims

  5. Issues and Dilemmas • There are no prevalence studies in UK and NA and often this kind of sexual assault is not recognized in the examination room. There is no central reporting data base and where there is no holistic assessment many cases are missed or not diagnosed unless there are specific behaviors which raise the level of concern and suspicion. Unusual behaviors, unwillingness to bathe or to undress, increased anxiety around personal care, graphic gesticulations, are some examples which have been noted in the literature.

  6. Forensic Issues • Early adolescent maladjustment • Unstable parental relationships • Lack of respect for mother who were highly strung and overprotective, domineering and provocative • Gerontophobic tendencies • Anger rape where the authority person has to be conrolled, hurt or degraded • Symbolic or surrogate victims • Histories of violence and sociopathic tendencies.
