    2. Heart Failure

    3. Heart Failure Demographics Framingham: incidence of heart failure doubles for each decade of life after age 40 500,000 new cases annually One-year mortality 28% in men > 75 With heart failure the age-adjusted death rate is increased 5.5 times for women, and 8 times for men

    5. Standard of Care

    6. Heart Failure Study Design Hypothesis – A Telemedicine System will hospitalization and improve QoL for Heart Failure Patients 44 Pts with Class II, III or IV HF & Hospitalization within 6 months. Randomized to Standard Care or Telemedicine Followed for 1 year Primary Outcome - Hospital Days Secondary - Quality of Life

    7. Kansas City Cardiomyopathy Questionnaire (KCCQ) is a valid, sensitive, and disease-specific method of questionnaire dedicated for congestive heart failure patients. 25. Heart Failure Classification Class I (Mild) No limitation of physical activity. Ordinary physical activity does not cause undue fatigue, palpitation, or dyspnea (shortness of breath). Class II (Mild) Slight limitation of physical activity. Comfortable at rest, but ordinary physical activity results in fatigue, palpitation, or dyspnea. Class III (Moderate) Marked limitation of physical activity. Comfortable at rest, but less than ordinary activity causes fatigue, palpitation, or dyspnea. Class IV (Severe) Unable to carry out any physical activity without discomfort. Symptoms of cardiac insufficiency at rest. If any physical activity is undertaken, discomfort is increased.

    31. For each question answered, there are certain numbers/score. The worst is the situation, the lower is the score. 32. Show sample questionnaire from KCCQ that was implemented in our study in the Telemedicine research center.
