Mission The Patient Access Network Foundation (PAN) offers help and hope to people with chronic or life-threatening illnesses for whom cost limits access to breakthrough medical treatments Background Established in 2004, PAN is an independent, national 501 (c) 3 organization Founded to help reduce the financial burden of underinsured critically and chronically ill patients and provide hope for leading productive and healthy lives again Governed by an 11-member Board of Directors with expertise in medicine, healthcare administration, non-profit management, finance and public policy Who We Are

What We Do • PAN provides financial assistance to individuals who have insurance but lack the means to pay for out-of-pocket costs associated with life-saving specialty medications • Who Qualifies • Individuals with Medicare, Medicaid or private health insurance who meet disease-specific eligibility criteria • Amount of Assistance • Depending on the illness, between $500 - $15,000 in copayment assistance for a 12-month period • Disease Funds • Nearly 60 disease-specific programs covering a wide range of patient needs

Program Assistance Relationship Flow: • Patient eligibility criteria: • Myelofibrosis (MF) • Polycythemia Vera (PV) • Essential Thrombocythemia (ET) • Patient has insurance coverage and is prescribed a high cost specialty therapy • Patient's income must be at or below 500% of the Federal Poverty Level • Patient must reside and receive treatment(s) in the US and its territories • $7,500 per year with the ability to receive additional funding, if available Patient Application Approval Letter Application PANFoundation Approval Letter SP/Provider 7
