  9. Choosing your specialty • Basic sciences vs. Clinical • Ten variable factors to consider in specialty selection. • Special consideration for women. • Considering your practice option.

  10. Ten variable factors to consider in specialty selection. • Generalist, specialist or non of the above: • What type of doctor you want to become?

  11. Ten variable factors to consider in specialty selection. • 2. Intellectual content and clinical issues: • should love the intellectual content of specialty • Interest in: (clinical material – basic science – reading textbooks, journals) • e.g: • Clinical pharmacology + physiology= anesthesiology • You don’t like anatomy → stay away from: surgery-radiology-pathology • Don’t force yourself to love an area of medicine.

  12. Ten variable factors to consider in specialty selection. • 3. Amount of patient contact: • Minimal contact with pt: radiology - pathology • Long term relationship: IM – FM • Getting down & dirty: ER – Ob/Gyn – surgery • Focused physical examination: urology – orthopedic • Cleaner specialty (lots of patient interaction with minimal physical contact): psychiatry - ophthalmology

  13. Ten variable factors to consider in specialty selection. • 4. Type of patient encountered: • Ask yourself: would you thrive emotionally & professionally in doctor-patient relationship?! • ER: angry pt – kept waiting for hours • Pediatrics: demanding concerned parents, sick child • Oncologist (M, S, R): mortal disease & poor outcomes • Ob/Gyne: highly litigious group of pt

  14. Ten variable factors to consider in specialty selection. • 5. Prestige, status, and social expectation: • Should be your own choice • Think about your happiness & forget about others view of your chosen specialty. • Remember by putting aside external influences such as social prestige & others expectations, you will likely choose the right specialty & end up a much happier doctor.

  15. Ten variable factors to consider in specialty selection. • 6. Lifestyle considerations: • Lifestyle specialties?! • Flexible career • Controlled hours of practice • Less stress • Adequate family time • More regular schedules • Income commensurate to the workload Radiology Dermatology ER Anesthesiology Pathology Ophthalmology Neurology

  16. Ten variable factors to consider in specialty selection. • 7. Length of residency training: • Shortest program: clinical pharmacology 2 years • Other mostly 4 years except: • G surgery – Ob/Gyn – Pathology – ENT – Urology → 5 years • Some are 6 years: plastic surgery & neurosurgery.

  17. Ten variable factors to consider in specialty selection. • 8. Difficulty of obtaining a residency or fellowship position: • Be realistic. • Unfortunately, just because your heart is set on becoming a plastic surgeon, dose not mean it will be possible. • Take an honest assessment of your academic competitiveness. • You may need a backup specialty if the field you want is beyond your academic reach.

  18. Ten variable factors to consider in specialty selection. • 9. Future income & earning potential: • Some do not think about salary, but others do so, because they want to pay off their loans right away, they lean toward specialty with high starting salary. • Remember that you will practice in that specialty for the rest of your life, so financial factor should have a less influential variable.

  19. Ten variable factors to consider in specialty selection. • 10. Job opportunities and predictions about the specialist workforce: • When choosing your dream specialty, you should pay little heed toward its current or projected state of job opportunities. • Shortage can be changed rapidly by the end of your residency program.

  20. Special consideration for women • How do you envision your practice style? • Short patient interaction • life-long relationship • action oriented

  21. Special consideration for women • 2. Could you handle working in a predominantly male environment? • More technical ones; male club: • Orthopedic • Surgery • Ophthalmology • ER • Radiology

  22. Special consideration for women 3. Would you be able to take maternity leave, have children & raise a family? The happiest female physician –no matter the specialty- are the ones who had children

  23. Considering your practice option • Enter academic rather than becoming a private practitioner !! • Less clinical time • Majority of patient management done by supervised resident • Great job flexibility • Additional time for teaching & research

  24. Advice for you!! • Ask & see • … …
