  2. Hope Medical Clinic of Ypsilanti, MI • Non-profit founded over 30 years ago • Provides 11,000 free medical and dental visits to 3,700 uninsured, low-income patients • 100 medical/dental + 250 other volunteers • Lacks on-site specialized equipment to provide appropriate diagnosis and triage

  3. Making an On-Site Partnership a Reality • Formulation of plan(s) • Memorandum of Understanding – a team effort including: • Legal • Facilities • Human Resources • Medical Education • Public Relations

  4. Making an On-Site Partnership a Reality Patients remain patients of Hope Medical Clinic • Hope provides and maintains all medical records • All follow-up visits, tests and medications managed by Hope • Hope obtains free radiologic, laboratory and pathologic via Trinity

  6. HOPE @ UMHS On-site Operations • Saturdays • All volunteers part of Hope workforce • UMHS volunteers include: • Attending/resident physicians, medical students • Nurse practitioners and/or PA’s, nurses • Audiologists • Medical assistants, optical shop/tech • Clerical staff 6

  7. Hope@ UMHS Operational Highlights • Program began October 2010 in otolaryngology; first five clinics provided 74 patient evaluations and 60 procedures • 2012: 401 total visits; 50 of these patients enrolled in UMHS MSupport charity care program for essential follow-up care 7
