Your Next Steps Full Wellness Rewards Credit January 1, 2013 – July 31, 2013 All Wellness Rewards Steps completed between January 1, 2013 and July 31, 2013 will receive the discount beginning with the first payroll deduction on or after January 1, 2014. Partial Wellness Rewards Credit January 1, 2013 – Dec. 31, 2013 All Wellness Rewards Steps completed by December 31, 2013 will receive the discount beginning with the first payroll deduction on or after June 1, 2014. For Your Points (300 Max): • Enroll in www.myUHC.com • Go to your “Personal Health Record” • Take your “Health Assessment” (100 points) • Have a Biometric Screening (100 points) • Earn 100 more points through preventive screenings, Online and/or Telephonic Coaching For Your Spouse/Partner Points (600 Max): • Enroll in www.myUHC.com • Go to your “Personal Health Record” • Take your “Health Assessment” (100 points) • Have a Biometric Screening (100 points) • Earn 100 more points through preventive screenings, Online and/or Telephonic Coaching
