  1. WachstumshormonersatztherapiebeimErwachsenen:AusserSpesen....WachstumshormonersatztherapiebeimErwachsenen:AusserSpesen.... Emanuel Christ, Universiätatspoliklinik für Endokrinologie, Diabetologie und Klinische Ernährung Inselspital, Bern

  2. D. Kappeler, European Congress of Endocrinology, Budapest, May 2007 .....“weil Du ein ewig Gestriger bist“...

  3. Atherosclerosis and the CV-System The effects of treatment with recombinant human growth hormone on body composition and metabolism in adults with growth hormone deficiency. Salomon F et al. .....The meanlean body mass increasedby 5.5 kg, and thefat mass decreasedby 5.7 in the group treated with growth hormone.....Fasting plasmacholesterol levels were lower in the rhGH-treated group than in the placebo group, whereas plasma triglyceride values were similar We conclude that growth hormone has a role in the regulation of body composition in adults, probably through its anabolic and lipolytic actions. Anti-Ageing Salomon F et al., N Engl J Med 1989

  4. Beneficial effects of growth hormone treatment in GH-deficient adults.Jorgensen JO et al. ...mean muscle volume of the thigh was significantly higher after GH than after placebo .... Growth hormone caused a small increase in theisometric strengththe quadriceps muscles and a significant rise in exercise capacity ....The heart rate both at rest and after maximum exercise was low during the placebo period and increased significantly during GH treatment. Blood pressure and echocardiographic wall mass of the left ventricle did not change during the study. These findings suggest that GH, in a conventional replacement dose, has several potentially beneficial effects in GH-deficient adults and therefore encourage future long-term trials. Exercise performance and CV system JO Jorgensen et al, Lancet 1989 Doping

  5. WH-Regulation

  6. WH Metabolismus Wachstum Protein: anabolism Lipids: lipolysis CHO: insulin resistance Lamberts et al., Science, 1997, 278; 419

  7. SMR erhöht (CV-Ereignisse) • Hypophysen-Insuffizienz • Craniopharyngioma • Hypogonadismus • Radiotherapie

  9. Geeigneter Patient Childhood-onset GHD (organic, genetic, idiopathic) Adulthood-onset GHD

  10. Symptome und Zeichen Erhöhung der Fettmasse (viszeral?), Verminderung der Muskelmasse (BMI 25-30) Verminderung der Leistungsfähigkeit (Alle anderen Hypophysenachsen ersetzt, wenn nötig) “Quality of Life”

  11. Labor (Leichte) Dyslipidämie: LDL-Cholesterin-Erhöhung „pro-immflammatorischer Status“ (hsCRP...)

  12. Körperzusammensetzung childhood-onset (CO) vs adulthood-onset (AO) Attanasio A et al, 2002, JCEM

  13. WH und „peak bone mass“ 23-25 Jahre

  14. Diagnose WH und IGF-1-Konzentrationen isoliert NICHT aussagekräftig Ausfälle von anderen hypophysären Achsen hilfreich Standardisierter Stimulations-Test (ITT; GHRH-Arginin) Zuweisung an den Endokrinologen

  15. Wirkung der WH-Ersatz-Therapie Symtopme und Zeichen Reduktion der Fettmasse (v.a. Waist) Erhöhung der Muskelmasse, Erhöhung der BMD Verbesserung “quality of life”

  16. - WH + WH 5-10% reduzierte Leistungsfähigkeit + 5-10% Cuneo et al., 1991 Christ et al. ENDO 2005 WH-Mangel und WH-Ersatz-Therapie

  17. Atherosclerosis (NEJM 1989) Exercise (Lancet 1989) Preload Lipids Body composition (IR) Oxygen Transport CVS Cardiac pump fonction Coagulation Factors Afterload Inflammatory Markers Endothelial function

  18. Wirkung von WH-Ersatz-Therapie – Meta-Analysis Maison et al. 2004, JCEM

  19. Polygonia interrogationis WH-Ersatz-Therapie und Frakturrate WH-Ersatz-Therapie und Mortalität

  20. Applikation von Wachstumshormon (1/Tag, abends sc., Eintitrierung)

  21. Leichte periphere Oedeme (5-18%) • Muskuloskelettale Schmerzen (Gelenke) • Carpal Tunnel Syndrome (2%) • Glucose Stoffwechsel (4% Diabetes mell.) Dosis-Abhängigkeit • Retinopathie (ohne Diabetesanamnese) • Intrazerebrale Hypertension (benigne) • Gynecomastie ohne Galaktorrhoe

  22. Rezidiv (adenom), Neoplasie-Risiko • Keine gesicherten Hinweise, dass WH-Ersatz-Therapie beim Erwachsenen das Risiko erhöht • (2 pädiatrische Patienten: Leukämie ? Colorektales Karzinom ? und Hodgkin ?) • Residual-Tumorgrösse bleibt im Allgemeinen stabil (CAVE: hoher Prozentsatz bestrahlt) • Active neoplasie = KI

  23. Monitoring Monatlich während Titration Klinisch (waist) IGF-I, Glucose (BIA) Lipid Profile (FT4, FT3, Cortisol) In „steady state“ (3-6 Mt.)

  24. Zusammenfassung (1) • Geeignete Patient für WH-Ersatz-Therapie (Klinik, Diagnose) • Patient mit schwerem WH-Mangel (HP-Op) • Klinik: Leistungsintoleranz, Körperzusammensetzung, QoL • Diagnose (Endokrinologe) • Welche Wirkung/Benefit kann erwartet werden? • Verbesserung der körperlichen Leistung, Körperzusammensetzung, QoL • Verbesserung des LDL-C • Welche Risiken hat die WH-Ersatz-Therapie? • Periphere Oedme häufig • Im Allgemeinen sehr gut verträglich • Cave: aktive Neoplasie, Blutzucker • Welches Therapie-Regime sollte gewählt werden und welche Parameter sollten monitorisiert werden. • Langsame Eintitrierung (Geschlechtsabhängig) • Monitorisierung: Klinik, Glucose, IGF-1, stady state Lipid-Profil

  25. Zusammenfassung (2) • „Peak bone mass“ • Response: geschlechtsabhängig • Therapie-Vorschlag bei entsprechender Klinik und Labor für eine begrenzte Zeit, dann Re-Evaluation • Wir retten keine Leben

  26. Ausser Spesen...? • Nein….. • Subjektives Ansprechen: unterschiedlich ca. 1/3 – 2/3

  27. Merci
