  7. Overview - Rx Changes in design from 2012 to 2013 In the HDHP, you will pay the full prescription cost until the annual deductible is met, then co-pays and co-insurance apply. Your prescription drug plan administrator is: Medco/Express Scripts

Overview – Flexible Spending Accounts Health Care FSA (HC-FSA) • Save money for out-of-pocket health care expenses • Set aside between $100 and $2,500 per year in before-tax paycheck contributions • For eligible expenses incurred between January 1, 2013 – March 15, 2014 (including 2½ month extension) • If selecting the High DeductibleHealthPlan, participation in the Health Care FSA is limited to eligible dental and vision expenses only. Day Care FSA (DC-FSA) • Save money on eligible day care related expenses • Set aside between $100 and $5,000 per year in before-tax paycheck contributions • For eligible expenses incurred between January 1, 2013 – December 31, 2013 Care$ • The Care$ benefit is unchanged in 2013 • First Data subsidizes eligible day care expenses for employees making less than $40,000/year. • Annual contribution is $720 if the employee also contributes to the dependent care flexible spending account or $480 if the employee only elects Care$ but no DCFSA. • Employees must actively enroll in the DC-FSA in 2013 to receive the higher Care$ benefit. • DC-FSA automatically enrolls them in Care$, if eligible. Enrollment does not carry forward – employee must actively elect participation to receive the benefit.

Overview – Healthcare Reform • Prior provisions implemented • Increasing Dependent eligibility age to 26. • Eligible expense definition for Health Care Flexible Spending Account. • Eliminating pre-existing condition exclusions from the medical plans. • Preventive care benefits with no employee cost sharing. • 2013 requirements to be implemented • Reporting the value of employer contributions for health insurance on IRS Form W-2. • Issuing uniform summary of benefit coverage for all medical plans. Located at www.FirstDataBenefits.us • Capping limit for Health Care Flexible Spending Account at $2,500. • Changes to women’s preventive health coverage, which includes FDA-approved contraceptive methods implantable devices, sterilization procedures and patient education, among other services with no employee cost sharing.

Overview – Take Action Take Action - Passive or Default Enrollment Is Not Advised • If you take no action at enrollment: • $900 Spousal Surcharge is applied when medical coverage is provided for a spouse or domestic partner (even if surcharge does not apply to your situation – any payroll deductions will not be refunded) • Wellness Incentives worth up to $650 may be forfeited • $200 Health Assessment Incentive (for HRA completion) • $250 Tobacco Free Incentive if tobacco free for 12 or more months • $200 Health Screening Incentive (for completing 2012 screening) • Not eligible for additional tax savings through Flexible Spending Accounts • May not be enrolled in coverage for plans that you want

Wellness Incentives $200 Health Assessment Incentive • To earn the incentive, COMPLETE a Health Risk Assessment (HRA) on SimplyWell’s website from November 5-15, 2012 • If employee elects to participate in medical coverage, employee must complete the online health risk assessment during enrollment • If employee and spouse/domestic partner elect to participate in medical coverage, both must complete the online health risk assessment during enrollment • If employee does not enroll in medical coverage, employee must complete the online health risk assessment $250 Tobacco-free Incentive • CERTIFY tobacco-free status on Your Benefits Resources (YBR) enrollment siteif tobacco free for the past 12 months and enrolled in a First Data medical plan • Incentive available ONLY to member electing medical coverage $200 Health Screening Incentive • COMPLETED an onsite health screening during Q1 2012, or • COMPLETED a Physician Upload form including biometric results and submitted to SimplyWell by March 31, 2012, or • COMPLETE biometric screening through annual physical. There is still time to earn for 2013 – if you’re annual physical completed in 2012 included biometrics, ask your doctor to fill out a Physician Upload form and send to SimplyWell by December 5, 2012 for 2013 credit Incentives are prorated per pay period and paid as earnings on each paycheck. While designed to offset medical plan premium deductions, only the tobacco-free incentive is not taxable. Health Assessment and Health Screening Incentives are applied to taxable earnings.

YBR - Spousal Surcharge • During enrollment: • Indicate if the surcharge applies. • If no action is taken but you provide medical coverage to a spouse or domestic partner, the surcharge will be applied. • 3. Certify the correctness of your response to the surcharge. Any changes in status during the year would result in a life event which allows you to make a change.

YBR - Wellness Incentives • There are several components to earning the incentive credits. • $200 Health Assessment Incentive includes 11/15/12 deadlines to: • Register on SimplyWell Web site • Complete the online HRA • Covered spouse completes HRA • $250 Tobacco-Free Incentive includes11/15/12 deadline to: • Certify your tobacco user status • $200 Health Screening Incentive • If you successfully completed Health Screening requirement by • December 5th, this incentive will begin appearing with your 2013 paychecks

YBR - Guided Enrollment • Once you’ve reviewed your benefits and are happy with your selections – Simply select the Complete Enrollment button. • After completing enrollment, the Submitted Successfully Page will appear which includes Required Follow-ups: • If Life coverage requires EOI – Complete Form requirement • To earn Health Assessment – Complete Requirements to link to HRA • If your spouse is enrolled in a First Data medical plan they will need to link to www.simplywell.com. • You’ll be eligible for incentive credits beginning in January if you complete all of these requirements by the enrollment deadline.

  28. Enrollment Checklist Take Action! Your Enrollment Checklist • Remember these dates: November 5–November 15, 2012. If you don’t enroll by November 15, you will be assigned coverage based on your current election or the applicable default option, including spousal surcharge and tobacco user status. • Take advantage of the Wellness Incentives available to you—up to $650 for the year. Your HRA must be completed by November 15, 2012. • Review the spousal surcharge requirements. If you enroll your spouse/domestic partner and do not opt out, you will be defaulted to “spousal surcharge applies,” and the $900 spousal surcharge will automatically begin being deducted on a per-pay-period basis from your first 2013 paycheck. Deductions will not be refunded. • Consider FSAs to save on taxes and manage expenses. Remember, participation in FSAs and Care$ Benefits (if eligible) does not roll over each year. You must actively enroll to have a Health Care or Day Care FSA in 2013. If you are in the HDHP for 2012 and stay in this plan for 2013, your health savings account (HSA) contribution amount will stay the same if you do nothing. • Select the appropriate dependents for each coverage option. You may select specific dependents for each option, so don’t assume all dependents are covered under each plan. First Data requires you to provide documentation showing that enrolled dependents meet eligibility requirements. Children are eligible for medical, dental and vision to age 26 (to age 19 or 23 if a full-time student for child life insurance). • Be sure you and each of your dependents enrolled in a medical or dental plan have a valid Social Security number on file in Your Benefits Resources. Dependents without an SSN on file cannot participate in First Data benefits. • Choose wisely. The choices you make during annual enrollment (or default to if no action is taken) will take effect on January 1, 2013, and will stay in effect through December 31, 2013. You may make midyear changes only if you have a qualified status change, such as marriage, divorce, birth or adoption of a child.
