Bradford & Airedale CCC Set up in August 2014 Successful multiagency partnership Operates a dynamic problem solving model Has achieved major changes in crisis MH services
Life - BCCC 1 EDT worker & 1 crisis worker at night Regular OOA placements & full wards No OOA urgent telephone line Police cells used regularly A and E breaches GPs frustrated AMHPs overstretched
How did we break the mould • NHS committed to First Response/Acute care restructure • LA/Police invited to be a partner • CCC format set up • CCGs, Hospital Trusts & Vol sector brought on board • Whole system approached developed
Why has the CCC model worked? • Cuts and change created a vacuum • The right people around the table • The right attitude to change • An action plan that is actually a vision for the future
Achievements so far …Services • First Response 24hr Crisis service • Intensive home treatment • A and E Liaison team • Police Hub team & Police Triage • Sanctuary Project • Integrated Partnership • Social care alternatives
Achievements so far …Outcomes • No Acute OOA placements 9 months • Reduction in A and E • Reduction in s136 • Reduced average length of stay • Alternatives to hospital for people not needing acute care • Diversion from custody
Partnership Working • Partnerships have been a key enabler • True multi agency committed representation • With a shared vision and objectives • In depth knowledge of Bradford across partners which has had a powerful impact in moving forward
The next stage … • Integrated Crisis Care Pathway • UEC Vanguard • Children and YP crisis pathway • Suicide prevention • Criminal justice
The Sanctuary • Part of Whole system approach • Integrated working between First Response & MIB • Developing pathways between medical & social models of health LTC • Demonstrating Asset based thinking across health (ABCD)
The Sanctuary at Mind Referral • The Sanctuary provides a safe space for adults experiencing mental distress. • Gatekeeping by Crisis service • Opening hours: Monday-Sunday 6pm-1am
Outcomes so far … • From May 18th to 7th Nov 2015 396 visitors • First responders have only needed to come out on 6 occasions • Working with two of the top five frequent attenders at A&E in Bradford • No one has accessed out of area beds • 85% of 338 visitors stated that they would have gone to A&E as nowhere else to go
The next steps • CCG impact evaluation • Applied for £100,000 of added value investment VHSF & LSF to develop staff & volunteers in community development & build community resilience • Peterborough & Cambridge Mind interested in duplicating model
