Stop and Correct Cards • We have decided to rebrand our Near Miss Reporting initiative and change it to “Stop and Correct” • Since starting the Near Miss Reporting initiative we have discovered that Stop and Correct reporting is resonating with our employees (i.e. they are reporting Stop and Corrects with much more frequency than “Near Misses”) • Additionally, we have found that most contractors have started Stop and Correct reporting and as a result, Vale PMO has requested that regular “Stop and Correct Reporting” is part of the “Contract Specific Safety Plan” submitted to Vale • Because of the above, we have created new Stop and Correct Cards. The cards are identical to the Near Miss Cards except that the front of the card now states “Stop and Correct” and has the definition. • 1000 Cards have been printed and are available for you to use. Additionally, the electronic version will be available on our Extranet Site without a logo. ¹ Content in the footnotes may include data or information that cannot be separated from the main content of the text. This supplementary information should respect the size of the slide and follow the Arial 9 in grey format.

Stop and Correct Cards • Vale PMO Requirement: • 1 Stop and Correct (minimum) per shift per job to be submitted as part of the Contract Specific Safety Plan. • Either the Vale provided cards can be used or the Contractors own forms. • Vale Representatives will report weekly the number of Stop and Corrects on Vale to Johanne Rochon who will track the number and the contractor. To be reported via the “Daily Report” (NAV-TP-0083) each Monday for the previous week. A spot has been added to this report for this use. • This will be reviewed at the Contractor Safety meeting in March 2016. • Contractors will be encouraged to use Stop and Corrects as Safety Shares at these monthly meetings. ¹ Content in the footnotes may include data or information that cannot be separated from the main content of the text.
