  1. Monthly Audit of 50% or minimum of 10 patients (variation dependant of Provider type) Report Monthly to Team and Service Leads of Division (business Unit) Report into Professional Groups and Committees as per Governance Arrangements Member of Team conducts the audit and enters directly into the testyourcare web based system or into the paper based system. (If data entered onto paper base system the data is entered into the web based system by the collecting member of staff ASAP). Member of Team conducts the audit, populates the data sheet as per the paper based system (and forwards to a member of the teams support staff to enter directly Into the web based system ASAP) Once data processed, the Audit Specialist runs the reports and forwards onto the Team and Service Leads Data scrutinised and corrected by Service Leads and RAG status agreed. This data enables a broad picture to be available at a glance. This picture includes service scores and whole divisional ( business unit) RAG status sent out to all Ward Sisters/Team Leaders on a monthly basis to discuss at team meeting & results displayed • Scores from the indicators (criteria) are forwarded to Governance & Risk Team • (or equivalent) for inclusion in the Trust monthly performance report which goes to: • Management Divisional Team Meeting (DMT) • Trust Board- (quarterly- monthly by exception) • Commissioners-( as per local agreement) • This data includes scores, comparison with previous month and reason for • deterioration in scores where this has occurred, brief actions to improve Care indicators data analysed, investigated and discussed within Clinical Divisional Nurses/ Team Leaders. Any necessary actions is decided within the divisions ( business unit) with support from Lead Nurses/ Heads of Service.
