  2. Process for Screening Current/Prospective Foster Parents • Complete the Agency Letterhead to include requestor name, title and name of each person who is being requested to be screened. NOTE: The Form 44 (Financial Form) and Prior Service Report form previously requested for Prospective foster parents will NOT be required for the CPS Screening only. The forms will still be required for OPM to approve the home. • Complete the Child Abuse Screening Request form for ALL current household members who are age 18 years old or older. • Ensure that ALL boxes on the Child Abuse Screening Request form is completed. • Upload both the Agency letterhead and ALL applications to GA Score. You will only need agency letterhead per family. • You will still be required to email the cps email to inform the unit that the required documents have been uploaded to GA Score for Prospective Foster Parents only. The unit will receive the email alert from Ga Score for current foster home requests.

Invalid Requests • Missing CPS applications for current house hold members who are age 18 or older • Missing Agency Letterhead • Missing maiden name or previous names used • No DOB • Partial or No SSN • Previous names not legible • Partial or missing physical address (NO P.O. Boxes) • Missing previous address(s) within the past five years • Missing full dates for previous address(s) • Information on the application is not legible • CPS Application is not signed by the applicant • CPS Application signed by the agency

  6. WHAT IS IT? A stand-alone application that captures: Substantiated CAN findings Details about maltreatment Demographic information Alleged child abuser (maltreator) Victim child Parent/guardian/custodian of victim child Parent/guardian/custodian of minor alleged child abusers.

  7. HOW DOES IT WORK? Once investigation is approved in Georgia SHINES, case information is transmitted to the Child Protective Services Information System (CPSIS). Registry staff sends a certified notice to the alleged child abuser that his/her name has been entered into the CPSIS. The alleged child abuser can request an appeal to have his/her name being in listed in CPSIS. The requester has 10 calendar days of receiving the notice to appeal the findings. Courts that can facilitate appeals are: • Office of Administrative Hearings (OSAH) • Superior Court • Court of Appeals • Georgia Supreme Court

WHO HAS ACCESS? • Per law, listed are some of the following that will have access to request information from the CPSIS: • CPS investigators • DECAL • State/government agencies which license entities that have interactions with children or responsible for providing care of children • Licensed agencies which interact with children or responsible for providing care of children and the information is used for licensing or employment 5. Self Check 6. CASA program for the purpose of screening and selecting employees and volunteers

  9. Process for Screening Staff for Current/Prospective Employment • Complete the Agency Letterhead to include requestor name, title and name of each person who is being requested to be screened. Type of request being submitted. • Print a copy of the CPSIS screening request form from: https://gacar.dhs.ga.gov • Complete the CPSIS Screening Request form for each current or prospective staff member on his/her hire date. • Ensure that ALL boxes on the CPSIS Screening Request form is completed. • Complete the screening request information in the CPSIS • Upload both the Agency letterhead and ALL applications to the CPSIS portal. You will only need one agency letterhead per individual. • The results will be sent back to the requestor that is listed on the CPSIS application. • The requestor will receive an email alert with a confirmation number once the required documents have been uploaded to the CPSIS portal.

WHAT HAPPENS WHEN A CASE IS UNDER APPEAL? • Level 1: OSAH • Registry staff will notify the county of the appeal request for an OSAH review by the alleged child abuser • Use current referral process to notify SAAG and request his/her representation at the hearing • OSAH notifies the county of the court date and time • Meet with your SAAG to prepare for the hearing. • Level 2-4: Superior, Court of Appeals, Supreme Court • If a court overturns the Division’s finding, the Division can consider filing an appeal with one of the above courts for the alleged child abusers name to remain in the CPSIS and the substantiation be upheld.

EXPUNGEMENT OF MINOR’S RECORD IN REGISTRY • Minor child abuser may be removed from the Registry if: • He/she has reached 18 years of age, • More than one year has passed from the date the act was committed that resulted in the substantiation, • There have been no subsequent substantiations, and • He/she can prove by a preponderance of the evidence that he/she has been rehabilitated.
