    1. Ealing Learning Disability Partnership Board Health and Leisure Sub Group Draft Terms of Reference 1)Purpose of the group The Health and Leisure subgroup will work on behalf of the Ealing Learning Disabilities Partnership Board

    2. The sub group has been asked to put a plan in place which helps people get better Healthcare and stay fit.

    3. 2) The need for such a group. People who use services, their carers, Ealing council, Ealing PCT and voluntary organisations must plan services together. People with Learning Disabilities often have more health needs.They must have their say about: Their health. Their care. Their wishes. “Nothing about us without us”

    4. 3) What the sub group will do 3.1 Look at the problems in Ealing which stop people with Learning Disabilities get better healthcare, and find ways to overcome these problems. 3.2 Talk to those who provide health care and agree on ways to over come these problems. 3.3 Agree with those who support people with Long Term Conditions that they need to include people with Learning Disabilities in their work. 3.4 Agree with services that any standards they use in their service, people with Learning Disabilities expect those standards to apply to them as well. ( NICE Guidelines), (National Service Frameworks)

    5. 3.5 Meet with GP practices which can now get money for starting a Health Action Plan and agree on what people with Learning Disabilities should expect from them.

    6. 3.6 Discuss people’s hospital experiences with Ealing Hospital staff and agree on how to improve on the admission, the care and discharge from hospital. This should make life easier for the patient, carer and staff on the wards.

    7. 3.7 The sub group will find out what information and training carers and people with Learning Disabilities need so that they can help themselves more.

    8. The subgroup will arrange for the right people to give that information and training.

    9. 3.8 Subgroup will produce a plan for training people who will help others to get a Health Action Plan. ( Health Facilitator.)

    10. 3.9 Subgroup will make links with other groups such as: Transition Planning Person Centred planning Patient Public Involvement Expert Patient Programme Projects to reduce health inequalities Health Promotion Programmes. Leisure and Recreation

    11. 4 What we want to achieve: The subgroup aims to end up with local plans for achieving; • Person centred health care system. A shift from a sickness service to a health improvement service People with Learning Disabilities having a say about their health and their care. Improved access to services for all.

    12. 5) Membership User representative Carer representative GP Dr Kate Cabot) Psychiatrist ( Dr Shastri) Practice nurse (Anne Hamerton) Practice Manager ( Fionnuala O’Dennell) Active Ealing ( Lauren Templeton) Representative from Housing Providers Nurse Manager (Geoff Chimbani) User Empowerment(Mike Bloodworth) Other people will be invited to contribute to the group from time to time.

    13. 6 )Meeting The subgroup will meet once a month for one and half hours. Minutes will be taken. Nurse Manager and a service user will co-chair the group

    14. 7) Reporting The subgroup will present a report to the partnership Board in 2007.Small reports on how the subgroup is going on about its work will be made to the Service Manager and the Integrated Commissioning Manager (Learning Disabilities).

    15. 8) Informing others From time to time the subgroup will tell services, people who use them and their carers what the group has done and what it is working on. This will be done through newsletters.
