  4. What is a Medication Reconciliation? A medication reconciliation is the process of creating the most accurate list possible of all medications a patient is taking — including drug name, dosage, frequency, and route — and comparing that list against the physician’s admission, transfer, and/or discharge orders, with the goal of providing correct medications to the patient at all transition points of a patient’s care.   5. Why should therapists do this? We are the doctors’ eyes and ears inside the home. We have the opportunity to catch any errors, misunderstanding, and adverse side effects while being in the home. It is requirement for home health services We are liable to ensure the safety of all of our patients in all environments to the best of our ability within our scope of practice FULL RANGE PT & REHAB SERVICES

  6. What is needed? A list from the referring MD Discharge orders from hospital Admission orders from office Transfer orders from SNF or rehab. An accurate list of what the patient is taking gathered while in the home. FULL RANGE PT & REHAB SERVICES

  7. How to complete? Using list from referring MD and list compiled while in patient’s home, cross reference all medications looking for any discrepancies: Dosage Time meds are taken Missing medications Frequency of medications FULL RANGE PT & REHAB SERVICES

  8. What medications to consider? All medications should be taken into consideration, including: Prescription Over the counter Topical Supplements Eye drops FULL RANGE PT & REHAB SERVICES

  9. Discrepancies Found? In the event that a discrepancy is found, the referring physician must be contacted and made aware. You must do this within 24 hours of visit. 1st attempt must be made while in patient’s home during SOC. Can’t get a hold of the physician? Document each attempt, detailing date, time, and who the message was left with. FULL RANGE PT & REHAB SERVICES

  10. New medication added during POC? Each visit always ask the patient if they started any new medications If so, ask to view bottle, box, etc... to see what the medication is, the dosage, the frequency, etc... Contact referring physician to notify of or confirm addition. Complete verbal order with change of medications, detailing communication with physician. FULL RANGE PT & REHAB SERVICES
