Day Centre, Active Age, Mother +Toddler. Physiotherapist, Podiatrist, Social Worker, Cognitive Behaviour Therapist, Dietician.

  3. Driver for Role Expansion Personal + Professional Development Evidence Based Care To Provide Holistic Care To Our Clients Develop + Maintain Professional Competency. Role Expansion Immunisations, Cervical Screening Diabetes + Cardiovascular Care, Warfarin clinic, Phlebotomy Smoking Cessation, Nurse Prescribing

Client Case Study Review Female, 82yrs, Widow High Blood Pressure, High Cholesterol Smoker 10/day,smoking for 60yrs+ Type Two Diabetes Atrial Fibrillation on warfarin tablets Annual Influenzae vaccine. Attends day centre twice a week, this allows client to come for regular diabetic+ INR + lifestyle, BP, weight, medication review. Individual Care Planning with the Clients Consent, plus regular follow up to reassess care agreed.

Time constraints – at work and home Financial constraints e.g. Nurse Prescribing Programme: Non-HSE Employee, no access to funding, supported by GPs Colleagues Team Meetings to discuss Clients Care Educational Development Study Days Practice Nurse Association Practice Nursing Co-ordination Support Challenges / Supports
